Reports to: Program Manager, East Hub Day Program
Salary: $54,864 to $64,962, plus for some of these roles a $2,400 per annum stipend for on call mobile response
Schedule: Full-time; these positions include roles that are M-F day time, M-F including some evenings, and Tu-Sa. All M-F staff will work one Saturday shift per month.
Service Location: SMHTO East Hub Day Program, Coxwell and Danforth
Position Description: We are hiring for the above-named positions, working as part of a multi-disciplinary team in our out-patient Day Program with a set of responsibilities that include shared operation of the Day Program space, supporting service users with intakes for Day Program and Hub services, access to food and other basic needs, and group programming, as well as primary responsibility for providing case management and/or counselling services to an identified case load of clients. These staff roles provide specialized care focused on substance use, mental health, voluntary trustee services and housing access/support.
The SMHTO Day Program is part of The East Toronto HART Hub, which provides evidence-based healthcare, treatments, social services, and housing support for individuals struggling with drug use, mental health, housing and other related issues. The Hub is an initiative of East Toronto Health Partners Ontario Health Team (ETHP OHT) led by South Riverdale Community Health Centre and St. Michael's Homeswith support from Anishnawbe Health Toronto, Gerstein Crisis Centre, The Neighbourhood Group, Michael Garron Hospital, Alpha House, Fontbonne Ministries, The Salvation Army, 2-Spirited People of the 1st Nations, Woodgreen Community Services, Comprehensive Treatment Clinic and The Neighbourhood Organization.
The key responsibilities of these positions include:
being the case manager with primary responsibility for an identified case load of clients with substance use, concurrent disorders, housing and other challenges;
being the primary counsellor for an identified case load of clients needing assistance with substance use, concurrent disorders and related issues;
working as part of the Day Program team, including creating a welcoming space for service users, providing intake for new Hub service users, assisting with basic needs access, informal assessment, and brief interventions. All members of the Day Program team, will spend some time in the day program common space working with clients;
working with clients to develop, modify and implement individual Care Plans;
conducting formal assessments in the areas of substance use and other health issues;
facilitating group activities, including SMART Recovery;
implementing the STOP NRT Program for interested service users;
assisting service users to access the needed range of services within and beyond the Hub partnership;
working with our partner medical and other providers to ensure continuity of care;
Trustee Case Managers provide specialized support for service users who choose to join our Voluntary Trustee Program, including working with service users to budget and plan for needed financial expenditures and disbursing funds to service users as per Voluntary Trustee Program policies and procedures;
System Navigator Case Managers will work on other organizations program sites, providing mobile and/or site-based support to the service users of partner organizations such as Michael Garron Hospital, assisting them with access to services within and beyond the Hub Partnership; and
managing client records and completing data entry in our EMHWare client management system.
Knowledge, Skills and Abilities
Comprehensive knowledge of issues related to substance use, mental health, trauma and recovery.
Knowledge and understanding of community substance use and mental health case management services.
Knowledge of the spectrum of harm reduction and abstinence approaches to problematic substance use.
Understanding of and commitment to health equity, diversity and inclusivity.
Experience utilizing various approaches such as cognitive behavioral therapy, psycho-educational, and SMART Recovery models.
Strong skills with Client Management Systems.
Demonstrated ability to integrate current best practice and research evidence to support professional practice decisions and actions.
Extensive knowledge and skills in assessment, supportive counselling, case management and brief-intervention skills.
Experience and skills working with individuals affected by poverty, violence, homelessness, institutionalization (incarceration), stigma, racism and/or homophobia/transphobia.
Ability to work collaboratively within and across interdisciplinary teams and community partners.
Experience working within an anti-oppression and trauma informed framework.
Excellent interpersonal and communication skills.
Proficiency in the use of computers and various software applications.
Qualifications
University Degree in Social Work (BSW) or equivalent education and related work experience.
Eligibility for membership or membership in good standing of a regulated health profession an asset.
Demonstrated experience in screening and assessment, supportive counseling, case management and brief intervention.
Demonstrated experience in motivational interviewing.
Demonstrated experience with group development, facilitation and evaluation.
Additional training in screening, assessment and treatment related to concurrent disorder and trauma an asset.
Proficiency and ability to work in both French and English is an asset.
Ability to work in a language in addition to English is an asset.
