POSITION SUMMARY
The Ottawa Health system is an intricate network of healthcare providers, services, insurance coverage, and programs that newcomers to Canada, such as RAP clients, struggle to navigate owing to linguistic, cultural, and other difficulties. The Health Navigator in the RAP program will work with the RAP team to help clients understand and access the health care system.
MAIN DUTIES & RESPONSIBILITIES
Under the direction of the RAP Program Manager, the Healthcare Navigator will:
Act as a liaison between various health and social service providers in the Ottawa catchment area, benefiting from Ontario Health programs and services, and the client.
Work closely with the Ottawa healthcare system, including but not limited to Urgent Care and walk-in clinics, Para Transpo, as well as the larger health community, to reduce barriers, such as logistical difficulties, and promote continuity of care.
Identify reliable culturally-aware resources that meet GARs healthcare needs.
Conduct health intakes and enrollments.
Follow up on clients referrals from health care providers to support clients with some logistical, and other non-clinical barriers to accessing care, following care plans, or meeting goals.
As needed, escalate and coordinate with clinical staff and care teams (e.g., primary/specialty/hospital social workers).
Collect information on barriers to healthcare access and any other barriers associated with the social determinants of health and share it with key stakeholders in order to appropriately advocate for additional resources.
Organize and deliver educational workshops for refugees and other IFHP-Eligible populations about various healthcare topics with the goal of increasing health literacy.

QUALIFICATIONS
Post-secondary education in social services, or an equivalent combination of education and experience.
Minimum 2 years of experience working in a social service setting
Experience providing one-on-one support to clients.
Knowledge of services and resources available to assist newcomers in Ottawa.
Excellent interpersonal skills, including negotiation, mediation, and conflict resolution.
Experience in facilitating workshops and group sessions.
Ability to deal with confidential materials and matters.
Ability to work independently and with minimal supervision.
Works well in a multidisciplinary, culturally diverse workplace.
Experience working with Microsoft Office Suite.
A valid drivers license and access to a reliable vehicle is a strong asset.
Fluency in English is required. Fluency in other languages is an asset
