Position Title: Social Navigator (Outreach & Case Manager)
Background
The John Howard Society provides services that assist individuals that are at risk of or involved in the Criminal Justice System and those at risk of or experiencing homelessness, with integration back into the community through offering housing support services, in an effort to assist individuals with getting established in the community of Thunder Bay.
Position Summary
This position will provide case management services for clients experiencing homelessness.
This position will provide outreach services to clients living in the community using a wrap-around approach to service.
This position will support clients in developing skills for living independently (based on the clients needs) and assist clients in finding permanent or long-term housing in the community.
Priority will be those considered the hardest to serve living on the streets and in encampments, as well as clients transitioning from the JHS Residence.
Position Responsibilities (Synopsis)
- Provide case management and outreach services to clients
- Work collaboratively with each client to develop an individualized Service Plan/Plan of Care that identifies needs, goals, resources, and actions required to meet identified targets for clients
- Work collaboratively with the JHS Case Management Team to discuss programming needs, progress, challenges, strategies, etc. for each client assigned to this positions caseload
- Respond to client issues and concerns appropriately, as necessary and required
- Make referrals to community organizations as requested/required by clients; liaise with other service providers collaboratively to provide interagency case management, especially when dealing with individuals with very complex needs
- Provide client with necessary information and support in navigating different systems and social service connections needed to obtain vital resources that assist in addressing underlying factors contributing to the experience of homelessness (ex: identification, financial support, employment assistance, counselling, education, treatment, etc.)

Additional Assets
- Experience working with diverse populations
- Experience providing outreach services is essential
- Knowledge of Homelessness and systemic challenges facing this population
- Knowledge of legislation pertaining to housing and homelessness in Canada
- Knowledge of and experience working with individuals from diverse backgrounds
Qualifications
Training: Post-secondary education in a related field (i.e. Humanities Social Service Work, Social Work, Addictions Counselling, Family-Child & Youth Counselling, Native Counselling, Recreational Therapy, etc.)
**Experience will be considered in lieu of formal training**
Experience: Work related experience in homelessness, mental health, poverty, community development, or residentialhousing programs. People with lived experience are encouraged to apply.
Position Details
Contract Length: One (1) year
Full-time: 35 hours weekly
Schedule: Afternoon Evenings (Wednesday to Sunday)
Salary Range: $40,968 $44,772 (annual)
