Position Summary

We are seeking a Child and Family Outreach Navigator to join our team. This position will focus on proactive outreach and developing individualized care plans to ensure families have access to the resources and services they require to thrive.
In collaboration with the Family Wellness and Preservation Specialistand Child and Family Liaison, the Child and Family Outreach Navigator will actively engage with families in the community, identifying those who are facing challenges and may benefit from additional support. This role will involve building strong, trust-based relationships with families, conducting holistic assessments that consider emotional, physical, and spiritual well-being, and working collaboratively with various service providers to create comprehensive care plans that address the specific needs of each family. The Navigator will also be responsible for facilitating communication between families and service providers, ensuring families are empowered with the knowledge and resources necessary for their success.
This is a one-year Full Time Term Contract with an end date of March 31, 2026, with the possibility of extension.
What you'll do
Outreach and Family Identification
- Conduct outreach to identify families in need of support, particularly those experiencing barriers toaccessing services.
- Develop and maintain relationships with families in the community to build trust and encourageengagement in support programs.
- Collaborate with external organizations and service providers to identify families at risk and thoserequiring additional support.
- Organize and facilitate community events to raise awareness of available resources and support programs
Assessment and Care Plan Development
- Conduct holistic assessments to understand the needs and challenges of families, including emotional,social, spiritual, and economic aspects.
- Collaborate with families and service providers to develop individualized care plans tailored to meetthe unique needs of each family.
- Ensure that care plans include access to available resources such as healthcare, housing, education,and other community services.
- Regularly review and update care plans to ensure they remain relevant and effective, addressingevolving needs.
Coordination and Referral
- Serve as a liaison between families and community services, ensuring smooth referrals to appropriateprograms and resources.
- Coordinate with other service providers to ensure a comprehensive approach to family support andcare.
- Advocate for families in need, ensuring they receive timely and appropriate services and support.
Follow-up and Monitoring:
- Monitor the progress of families in achieving the goals outlined in their care plans.
- Provide ongoing support to families, adjusting care plans as necessary to accommodate changes intheir circumstances.
- Conduct follow-up visits or check-ins to ensure families are receiving the support they need and toaddress any concerns.
Documentation and Reporting
- Maintain accurate and up-to-date records of assessments, care plans, services provided, and follow-upactivities.
- Prepare reports for internal use and community stakeholders, ensuring compliance with policies andprocedures.
- Ensure confidentiality of family information and compliance with privacy regulations.
Collaboration and Teamwork
- Work closely with other team members, including healthcare providers, social workers, andcommunity leaders, to ensure the holistic support of families.
- Participate in team meetings and case reviews to share insights, discuss challenges, and identifysolutions.
- Attend relevant training and professional development opportunities to stay current with bestpractices in outreach and family support.
Liaison with External Agencies and Service Providers
- Establish and maintain strong working relationships with external agencies, organizations, and serviceproviders to ensure seamless coordination of services.
- Facilitate communication between families and external partners, advocating for timely and culturallyappropriate services.
- Stay informed about available programs, resources, and opportunities offered by external agenciesand ensure families are connected to these services.
- Participate in inter-agency meetings and collaborative forums to discuss family needs and ensureintegrated service delivery.
Other Duties
- Additional responsibilities as needed to support the community and uphold our shared values.
Working / Special Conditions
- Initial and ongoing employment is subject to submission of a clear Criminal Record Review Program to ensure the safety and trust of the communities we serve. This check will be conducted every five years.
- Initial and ongoing commitment to the values and guidelines of Sq'wqel (Seabird Island Band),including our policies, procedures, standards of practice, and laws, is essential. This commitmentsupports our collective work, the community, and the responsibilities of this role.
- Must possess a valid class 5 driver's license and have access to safe and reliable transportation, insured for community-based use, to effectivelyengage and support projects, work, and initiatives
- Flexibility in working hours may be required, including evenings and weekends, to accommodate theneeds and schedules of clients and community needs. Employees will be given as much notice aspossible to any changes in schedule.
What you bring
Qualifications
- Bachelors or Diploma in, advocacy, social work or a related field is required.
- At least 2 years of relevant experience working within Indigenous communities, focusing on Child andfamily.
- Proven experience in community outreach, preferably with Indigenous families and communities.
- Strong understanding of Indigenous cultures, traditions, and practices, with the ability to engagefamilies in a culturally safe manner.
- Experience conducting holistic family assessments and developing individualized care plans.
- Strong communication skills with the ability to work effectively with families, service providers, andcommunity leaders.
- Experience advocating for families and connecting them with appropriate services and resources.
- Ability to manage multiple cases and maintain organized records.
- Knowledge of privacy regulations and ethical standards in family services.
Preferred
- First Nations candidates are strongly encouraged to apply.
- Lived experience as a member of a First Nations community.
- Knowledge and understanding of Seabird Islands culture, traditions, teachings, community dynamics.
- Ability to respond quickly and professionally to community or colleague needs and inquiries.
- Excellent writing, editing, charting and verbal communication skills.
- Proven experience in implementing and managing family care plans.
Cultural Competency
- Deep understanding of Indigenous cultures, traditions, and contemporary issues.
- Understanding of intergenerational trauma and its impacts on Indigenous peoples and communities.
- Experience working within Indigenous communities and applying culturally safe practices.
