CASE MANAGER TRANSITIONAL CASE MANAGEMENT

Purpose of the Position: This member of the team has responsibility to carry out case management, psychosocial rehabilitation, crisis intervention and treatment for individuals living with serious and persistent mental illness under the clinical supervision of the team, the Team Leader or delegate. This team member has the clinical skills and experience necessary to assess, develop, co-ordinate, and provide a plan of care by way of recovery and support systems used throughout CMHAMPS and other community services. This team member supports opportunities for clients to increase or develop new skills to become more successful and satisfied in their daily living, learning, working and social environments of their choice with the least amount of professional help required. This team member will be an expert in engaging the community and developing a network of landlords, community service providers and clients in the District of Muskoka.
Education and Qualifications:
- A bachelors degree in behavioural science, social work or equivalent is required. Equivalence for the purpose of qualification means a university education in the social sciences, which includes or is supplemented by practical courses in these disciplines.
- One year of experience working with persons who have mental illness and/or developmental disability and/or substance use issues plus experience in providing one or more of the following;
- supportive counselling
- case management
- crisis intervention
- Knowledge of mental illness and of the health and social service program
- Ability to work professionally, autonomously, collaboratively, and respectfully within a team environment and with a range of internal and external stakeholders in a rural setting.
- Familiarity with Psychosocial Rehabilitation principles and practices and the concept of recovery.
- An understanding of the concept of recovery and a commitment to work for its ongoing expression in the service system.
- A commitment to the rights of those experiencing serious mental illness/addictions.
- Knowledge and awareness of Indigenous culture and language considered an asset.
- Excellent communication, organization, and interpersonal skills
- Working knowledge of Microsoft Windows, the Internet, and data management software
- A willingness to serve clients within the communities of the District of Muskoka and Parry Sound
- A valid Ontario G or G2 drivers license, a safe and reliable means of transportation, andappropriate insurance for work purposes on your vehicle with a minimum of $1,000,000 in liability coverage.
- Bilingual in French/English language considered an asset
- Work schedule may include days/evenings/weekends and statutory holidays.
Salary scale: $29.26 - $38.32 per hour commensurate with qualifications and experience
