The Permanent Supportive Housing (PSH) Integrated Health Services team offers wrap-around health care supports for people living in partnering PSH programs in Guelph. PSH is a long-term, affordable housing with 24/7 flexible, voluntary support services to help high-needs individuals who experience chronic homelessness and require intervention to assist maintain stable housing and to live successfully in independent or semi-independent housing, with housing staff onsite.

The OT will conduct assessments, intensive treatment, rehabilitation, and develop support plans for residents living across the 4 sites. The OT will work directly with residents to support the implementation of resident plans, with ongoing consultation and re-assessment, as residents needs change. The OT will also include PSH staff in the development and implementation of residents plans so that PSH staff have the information and tools needed to support residents to implement their plans and build key skills. This position will work at multiple sites in the community.

PRINCIPAL JOB DUTIES

  • Complete assessments with residents related to their current abilities to engage in a variety of daily living activities, including managing their apartment, taking medication.
  • Assessment may include functional abilities assessments, Activities of Daily Living (ADL), Instrumental Activities of Daily Living (IADL) assessments and sensory profile assessments. Help clients meet sustainable housing goals
  • Recommend strategies and treatment plans
  • Collaborate with the residents other supports (i.e. case managers, outreach workers) to coordinate supports needed for client to be successful in their housing placement and to achieve their health goals
  • Consult with other team members on the client needs and care
  • Build relationships with clients and community partners
  • Assist with the development, monitoring, and analysis of continuous quality improvement plans
  • Support the evaluation of programs and work to identify and implement improvements
  • Work with and coach PSH program staff to support the implementation of residents plans

KNOWLEDGE, SKILLS & ABILITIES

  • Graduate degree in Occupational Therapy
  • Active member in good standing with the College of Occupational Therapists of Ontario
  • 3-5 years experience providing occupational therapy assessment, ongoing informal assessments, treatment, and rehab in a community mental health setting working with adults
  • Certification in First Aid and CPR
  • Proficient computer skills including Office Suite and EMR
  • Valid Ontario drivers license and access to a reliable vehicle
  • Lived experience is an asset
  • Demonstrated expertise in client-centred, trauma-informed care and understanding of the value of interdisciplinary care in a health care setting.
  • Solid team player with a demonstrated ability to work with flexibility and efficiency in a fast-paced environment.
  • Ability to work effectively with individuals with complex needs and involvement with multiple services and systems.
