Occupational Therapist
Permanent, 0.6-0.8 FTE (3-4 days per week)
The Upper Grand Family Health Team (UGFHT) offers the opportunity to join an innovative, community-based primary health care team serving the Arthur, Fergus, Elora and surrounding areas. We are looking for a talented team player to join us as we continue to provide a model of excellence in rural primary health care.
Position Summary As a member of an inter-professional team, the Occupational Therapist (OT) works to their full scope of practice to provide primary care assessment and intervention for clients of all ages. The Occupational Therapist is an important clinical team member supporting primary care in our community through a varied clinical portfolio including, but not limited to: chronic disease management, geriatric care, memory clinic, chronic pain management and individual assessments.
The successful candidate will work collaboratively with other members of the interdisciplinary care team at the Family Health Team, to provide a variety of health promotion and disease prevention programs to our clients.
Requirements:
- Degree in Occupational Therapy from an accredited university program
- Membership in good standing with a regulated association with the College of Occupational Therapists of Ontario and liability insurance coverage are required
- Experience in falls prevention and home safety, functional assessments, chronic disease management, mental health, chronic pain, musculoskeletal assessments and management, and group education preferred
- Additional training with geriatrics and or memory clinics preferred
- Ontario Ministry of Health and Long-Term Care Assistive Devices Program (ADP) Authorizer status for ambulation aids considered an asset
- Knowledge of client-centered philosophy, change management, basic principles and practices of community service delivery, short-term treatment options and chronic disease management models
- Knowledge and proficiency in current, evidence-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction
- Demonstrated experience in a primary health care (family medicine) or community setting preferred
- CPR-C/ Standard First Aid an asset
- Valid Ontario Drivers License and access to own method of reliable transportation
Responsibilities:
- Clinical support for the programs and responsibilities connected to the role
- Comfortable completing visits in office, at patient homes, and using virtual care tools
- Determine the need for, and the implementation of health promotion and prevention strategies for families and individuals
- Maintain professional competence and remain current (e.g. evidenced-based care) through ongoing participation in professional development, quality projects, etc.
Skills and Abilities:
- Proficiency in the use of computer and communication system knowledge: Email, Electronic Medical Record systems (Telus PS Suites an asset)
- Demonstrated skills in assessment, program development, implementation and evaluation of health promotion, disease prevention and chronic disease management programs
- Strong communication skills which reflect the ability to work well in a team environment while maintaining respect and attentiveness to patients and fellow team members
- Ability to keep confidential all information regarding patients, health care providers, employees, business operations and clinic/team functioning
- Organizational skills and time management abilities to prioritize a wide range of job activities with accuracy, and complete tasks within agreed upon timeframes
- Problem solving skills to identify and effectively solve problems
