Compensation: $19.73/hour
Position Summary: Responsible for assisting the LPN in delivery of resident-centered care services on a shift- by-shift basis and in accordance with program goals and processes; through the Vibrant Advantage Philosophy of care; and evaluates effectiveness of high-quality care and service provision on an ongoing basis.
The following job responsibilities are essential for the success of this position:
- Ensures that comprehensive Resident Care Plans reviewed by the LPN are regularly updated, evaluated for effectiveness, and placed in appropriate locations.
- Participates in health and safety matters and resident care, coordinating the interdisciplinary team to ensure regular care conferences are conducted.
- Documents changes in care delivery resulting from care conference decision-making.
- Provides input on education planning and delivery, quality improvement initiatives, and program development.
- Communicates with the LPN regarding trends, evolving needs, and precedents that impact care and service delivery, recommending necessary program adjustments.
- Monitors and ensures residents' rights to privacy, dignity, and respect are consistently upheld throughout care and service delivery.
- Advocates for residents and supports families in maintaining meaningful relationships with their loved ones. Facilitates positive resolutions to complaints and escalates unresolved issues to the General Manager.
- Assists with resident move-ins, transfers, and move-outs within assigned areas. Coordinates with the pharmacy to address individual resident needs while incorporating input from residents and their families.
- Ensures resident electronic and health records are accurately managed by all team members.
- Ensures adherence to proper procedures and legal requirements regarding confidential matters, including resident, family, employee, and volunteer information, company data, and medication records.
- Ensures adequate supplies and resources are available to support program requirements.
- Attends all required or recommended educational sessions to enhance knowledge, skills, and abilities.
- Performs other duties as assigned by the LPN.
Qualifications/Educational Requirements/Certification Requirements
- Minimum of two years experience involving care and service delivery within a care environment preferred
- Strong desire to enhance the lives of seniors by leading and mentoring team members and coordinating care services around resident needs, abilities, and preferences.
- Computer literate and familiar with electronic health records including PointClickCare and eMAR.
