Clinic Nurse, Care Coordinator - Outreach


To provide preventative and primary health care services that promote mino-bimaadiwizin. Collaborate with interprofessional team members (including community providers) to implement programs that promote mino-bimaadiziwin utilizing both Indigenous and contemporary approaches in design.


The Clinic Nurse Care Coordinator works within an interprofessional team comprised of registered and non-registered healthcare professionals, both directly and under the facilitation of medical directives, to increase the capacity for screening, early identification, case management and vaccination services to the population seen in outreach.
The Clinic Nurse Care Coordinator provides primary health care services with an emphasis on the prevention and treatment of Blood Born Infections within the professional scope of practice established by the College of Nurses of Ontario.


This position is part of the Ontario Nurses Association Bargaining unit.


NOTE: Indigenous traditional practices involve cultural ceremonies and burning sacred medicines including tobacco, sweetgrass, sage and cedar within the work setting.
QUALIFICATIONS:
current registration as a Registered Practical Nurse with the College of Nurses of Ontario;
willingness to become member of other professional bodies to enhance practice;
work to a full scope including performing delegated acts through medical directives;
working knowledge of provincial immunization schedule and experience with the administration of vaccines an asset;
training and experience with venipuncture procedures and wound care an asset;
experience and demonstrated ability to work in a collaborative, de-colonial, trauma-informed, and client-centered approach to support WNHAC values in meaningful ways;
deep understanding and experience of Indigenous culture, values, and perspectives as well as traditional and contemporary health care practices, and wholistic health and wellbeing; ability to speak and understand Anishinaabemowin a definite asset;
experience working in an Indigenous organization; understanding and/or willingness to learn of Indigenous culture and values;
proof of immunization in compliance with policy requirements is mandatory;
valid drivers license, own vehicle and willingness and ability to travel required;
criminal records check and current drivers abstract required;
commitment to ongoing training and professional development relevant to job requirements; and positive attitude and capacity to act as a healthy lifestyle role model.
