In partnership with NS Health, PRAXES operates the Mentor Clinic: a demonstration clinic for the health home model of primary care, which involves a well-supported multi-disciplinary team with the patient as the center point. Relationships are a key element of the health home, and every role has a vital and valued part to play in the success of the team. Mentor Clinic is separate from but contracted to NS Health. It is publicly funded there are no patient fees. Mentor means experienced trusted advisor. The broad-based inter-professional and layperson clinic will 1. mentor one another 2. mentor patients 3. patients will mentor us.

Licensed Practical Nurse Role:


The Licensed Practical Nurse (LPN) uses their clinical knowledge and skills within their scope of practice to support the patient and provider through the various stages involved in the primary care process. With a new and developing model, the role of the LPN will change and evolve. Continuous learning and feedback opportunities will be provided for all members of the Mentor Clinic team.

Note: This position is based in Halifax Nova Scotia and so the successful candidate will need to be physically located in Halifax. This role will involve occasional evening and weekend shifts.

Expected Job Outcomes:

Patient Care

  • Pre-visit Staging of the Patient:
    1) Triaging and assisting/supporting Family Practice Nurses, Nurse Practitioners, Doctors and inter-professional staff with visits.
    2) Research all reports, lab work, letters and medications and enter the chart to support patient visits.
  • Clinical Visit Supports:
    1) Assist providers with taking patient vitals (height, weight, BMI, blood pressure) and conducting any pre-work as appropriate to patients symptoms, attributes or reasons for visit (e.g. glucose test, urinalysis, temperature, vision, lung capacity, etc.).
    2) Provide charting supports to team members during visit as needed.
    3) Provide patient education and documentation to support patient wellness, self-care, and information about tests/procedures.
  • Follow Up Care:
    1) Follow up on tasks assigned by clinical team members (labs results, referrals, calls to health care providers etc.)
    2) Review charts to identify patient screening requirements as per protocol (e.g. mammogram, PSA).

Mentorship, Education, and Process Improvement

  • Develop positive working relationships/partnerships with other disciplines, agencies and health care professionals in order to promote coordinated service, quality care and access to appropriate resources for patients.
  • Collaborate with members of an interdisciplinary health team such as family physicians, nurses, dietitians, occupational therapists, social workers and others to plan, implement, coordinate and evaluate patient care in consultation with patients and their families.
  • Participate in meetings and Continuing Medical Education sessions as required.
  • Assist with development and improvement of systems and processes used by all providers within the clinic.

Skills and Qualifications:

Must-have

  • Licensed Practical Nurse certification in Nova Scotia.
  • The ability to maintain employee confidentiality during interactions with staff.
  • Solid computer skills with a working knowledge of EMR and Microsoft Office.
  • The ability to work collaboratively with other health care providers.

Nice to Have

Clinical experience in a family practice environment.
