About the Role:

The Lets Grow Together Program provides individual and group counselling to individuals connected to Stonehenge Therapeutic Community and in conjunction with the Guelph Community Health Centre who need support with their substance use while pregnant and or parenting. This Counsellor role works as part of a team and is dedicated to supporting women who are pregnant or parenting.

Role Type: Full time, Contract (approx. 12 months)

Qualifications:

  • Completion of a University Degree relevant to mental health, addiction, or Social Services
  • Minimum one year of progressive experience in addictions or mental health; experience in concurrent disorders and trauma informed practice an asset
  • Previous experience in substance use disorder and history of working with children preferred
  • Registration with a professional college (i.e. OCSWSSW or CRPO) required
  • Lived experience related to addictions and/or mental health an asset
  • Experience within the homelessness serving sector an asset
  • CSC Clearance conducted by PWGS Canada
  • Vulnerable Sector Check
  • Standard First Aid/CPR, Level C
  • Valid G Drivers License (G2 or G) and Satisfactory Motor Vehicle Record
  • Reliable vehicle and willing to transport program participants
  • Must provide proof from Insurance provider of 1) at least $2 million in general liability and 2) acknowledgement of scope of vehicle use by Insurance provider

Role Overview:

Counselling Services:

  • Support the intake of clients through consultation and assessment
  • Develop and coordinate weekly programming for the Lets Grow Together program for pregnant and parenting women who are substance involved, and their children (ages 0-6)
  • Coordinate and deliver weekly support and education groups for pregnant and parenting women with substance use issues
  • Provide individual support and advocacy to group members as required, including liaising with Family and Childrens Services
  • Support the ongoing promotion and evaluation of the Lets Grow Together Program
  • Deliver clinical and support services for clients of the program that include:
    • Co-develop goals with program participants and monitor progress towards these goals in collaboration with identified supports (where appropriate)
    • Provide 1:1, as well as group facilitation counselling and psychoeducational interventions that support progress towards identified goals and enhance overall health and well-being, particularly in relation to mental health and substance use issues
    • Facilitate crisis planning and intervention as required
    • Utilize evidence-informed practices including (but not limited to) Motivational Interviewing, Stage-Matched Intervention, Trauma-Informed approaches, CBT and DBT
    • Support participants to maintain housing and develop employment/education, budgeting and life skills
    • Make referrals to internal and/or external program or services, as appropriate
    • Provide comprehensive and collaborative discharge planning
    • Facilitate community building with program participants and the Guelph communities including neighbors, community groups, resources, etc.

Consultation:

  • Provide clinical consultation to Guelph CHC, Midwifery Guelph, Family and Childrens Services etc.

Administration and Reporting:

  • Maintain appropriate confidentiality and documentation standards as required by the agency
  • Involvement in program evaluation, data collection/input into CaseWORKS
  • All other duties as required
