Who we are? We are a leader in First Nation Health. Our foundation is Indigenous Health in Indigenous Hands. We provide accessible, quality, culturally safe and holistic health care that supports and enhances the wellness of individuals, families, and communities. We work with our 11 First Nation community partners and one Indigenous Friendship Centre in the provision of primary health care, traditional healing, mental wellness and addictions, home and community support services and health promotion services. We have recently been approved as the first Indigenous led Ontario Health Team by the Ministry of Health.

The opportunity: Under the direction of the Minobimaadizing Addictions Manager, the Minobimaadizing Support Coordinator is responsible to provide clinical support to community members who access the Minobimaadizing Addiction Clinic. This role will focus specifically on the Medication Assisted Therapy program and support clients with their withdrawal management from substances, relapse prevention, transition, and aftercare.

What you need to bring with you:

Registered Practical Nurse College Diploma and current registration in good standing with the College of Nurses of Ontario is required

Current CPR and First Aid

Having knowledge of prescription drug abuse or polysubstance abuse would be an asset

Demonstrated knowledge and experience in using drug assessments and screening tools

Ability for conceptualizing and addressing individual addiction issues from a wholistic perspective

Demonstrated ability to develop and facilitate health teachings and presentations for health promotion and awareness related to prescription and illicit drug abuse

Cultural awareness and sensitivity training an asset

Excellent interpersonal skills, ability to work independently and within a team setting

Knowledge in the use of computers, various word processing, spreadsheets, and electronic medical records

Must be able to work flexible schedule including evenings and weekends

Must have a valid G Class drivers license and access to a reliable vehicle

Duties and Responsibilities

Complete initial intake forms with client

Complete Monitored Urine Drug Screening (UDS) and enter into patient chart

Assist in referrals to other agencies and treatment centres

Participate in client care plan and coordinate case conferences when needed

Maintain flow of clients during clinic hours and keeping the Physician informed of client arrivals and any specific

client needs

Participate in client appointments with Physician when requested

Any other related duties as required

Develop and facilitate health teachings and related to prescription and illicit drug abuse

Attend community events for program promotion

Provide educational presentations as requested

Participate in relevant community and regional committees for service integration and collaboration/partnership building
