The System Navigator to provide client-centered, community-based services to individuals staying at the Inn Out of the Cold Shelter, assisting them in navigating the health and social system supports available to them. The position will include individualized assessment and planning, monitoring and support, systems advocacy, and resource coordination.

Successful candidates must have a Police Vulnerable Sector Check (PVSC) completed within the last six months. In order to avoid delays in beginning a position, interested applicants who are in need of a PVSC are encouraged to apply for one as soon as possible.
WHAT WE'RE OFFERING:
- What you'll be making - Starting rate for this role is $26.74 per hour (Level 1 – Direct Service), with a wage differential, based on regulation ($29.57 per hour for Level 1 – Direct Service, Regulated):
- Successful candidates who are registered with one of the designated professional colleges (e.g. OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation
- Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($33.90 per hour for non-regulated, and $36.71 per hour for regulated)
- For internal applicants, grid level placement will occur as per the Collective Agreement
- When you'll work - 70 hours over a two week period, 9am-4:30pm Monday-Friday

HOW YOU WILL BE MAKING A DIFFERENCE:
- Provide and facilitate referrals to mental health and addiction resources as appropriate (e.g. referrals for psychiatric consultation, addiction medicine, mental health supports and referrals to internal or community resources)
- Assist and advocate for/with clients to navigate systems, connect to appropriate resources, obtain needed services and supports, and to complete referrals to external services or to adhere to court or probation conditions (where applicable)
- Assess situations requiring specialized services (e.g. Legal Services) and arrange appropriate linkage and support
- Assist client to secure safe and affordable housing
- Use standardized tools (GAINSS, OCAN, LOCUS, VI-SPDAT) and their results to help the client identify their goals and preferences
- Advocate on behalf of clients (e.g. with hospital and community services, landlords, boarding houses, police) to ensure the client's rights are not compromised
- Offer harm reduction and other strategies based on client's intentions and stated needs
- Willingness to attend to client who are encamped or sleeping rough to offer support, referral and intake at emergency shelters
- Provide harm reduction kits and tools to ensure safer consumption
- Assist clients, when required, to deal with environmental difficulties and to develop social living skills conducive to community living
- Encourage and facilitate personal growth and development towards recovery by providing individual skill teaching and counselling; motivate the client to obtain the necessary skills and behaviours through training from other sources
- Provide health and wellness support including participation in and/or co-ordination of Community Care Plans (CCPs), relapse prevention and planning, wellness/recovery planning, medication education, symptom and medication monitoring, referrals to specialized health services, and support with physical health concerns
- Provide needed information and education to key stakeholders about mental illness, addiction, coping skills, services, resources, self-help and consumer initiatives/supports
- Meet with client regularly to work on specific goals and adjust interventions until other permanent supports are in place
WHAT YOU'LL NEED TO APPLY:
Police Vulnerable Sector Check
Successful candidates must have a Police Vulnerable Sector Check (PVSC) completed within the last six months. In order to avoid delays in beginning a position, interested applicants who are in need of a PVSC are encouraged to apply for one as soon as possible.
We require the following qualifications:
- Post-secondary degree or diploma in a program relevant to mental health or social services
- (primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
- Valid Driver's Licence
- Vehicle insurance in compliance with CMHA TVAMHS policy
- Current, clear Police Vulnerable Sector Check (PVSC)
- Demonstrated ability to attend work regularly
We prefer the following qualifications:
- Current certification in Non-Violent Crisis Intervention (NVCI), ASIST Suicide Intervention, and/or Standard First Aid
- Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW), the College of Psychologists of Ontario (CPO), or the College of Nurses of Ontario (CNO)
- Minimum one-year full-time employment experience in delivery of mental health, addition, or community-based intervention services
- Working knowledge of the Mental Health Act, and an awareness of trends in the provision of support services to adults with serious mental illnesses and addiction
- Knowledge of IPAC Congregate care home guidance and l cal RBI Housing resource
- Working knowledge of mental health status exam information
- Extensive knowledge of the support/treatment services network in Elgin County, and or regional services commonly accessed by the program
- Knowledge of the Residential Tenancies Act
- Understanding of the Landlord Tenant Board mechanism
- Understanding and working knowledge of Landform Form N1-N13
- Employment experience delivering services to individuals with problematic substance use and/or contact with the justice system
- Bilingual both official languages, preferred; préférence sera accordée aux candidats qui sont compétents dans les deux langues officielles du Canada
PHYSICAL REQUIREMENTS:
- Standing and walking for extended periods of time;
- Able to climb stairs, as the homes supported are not fully accessible.
