ROAD TO RECOVERY (R2R) HOME CASE MANAGER

Hours of Work: 35 hours per week

Purpose of the Position: The R2R Case Manager is part of a multidisciplinary team and is responsible for providing support to residents within a semi-independent congregate living environment. The Case Manager is responsible for carrying out case management, psychosocial rehabilitation, crisis intervention and treatment for individuals living with mental illness and/or addictions; specifically supporting residents of the R2R Home. This team member has the clinical skills and experience necessary to assess, plan, develop, co-ordinate, and provide treatment by way of rehabilitative and supportive services in a multi-disciplinary and/or collaborative environment. As a member of a multidisciplinary team and with strong clinical-case management abilities, the member will provide recovery-oriented supports within the congregate living setting. The member will also support the daily operations of the R2R Home as required. Awareness and application of psychosocial rehabilitation and recovery-oriented services is an asset.
Requirements of the Position:
- Bachelors degree in a human service/health science discipline relevant to the position. Equivalency may be considered for a combination of educational experiences. Examples of equivalency include;
- Mental Health related diploma with relevant work experience PLUS registration with a regulated health professional college
- One year of experience working with persons who have mental illness and/or developmental disability and/or substance use issues preferred, plus experience in providing one or more of the following;
- supportive counselling
- case management
- crisis intervention
- Manage internal and external relationships, to have and demonstrate empathy and compassion, to be aware of emotional intelligence, conflict resolution, and the understanding of the feelings, ideas and beliefs of others.
- Indigenous cultural awareness and education is an asset.
- Bilingual in French is an asset.
- Ability to communicate effectively, both orally and in writing.
- Tolerance of and ability to handle a heavy workload and competing demands and high risk situations.
- Ability to use and learn new technology (computers, software programs, phones, printer/scanner, alarm/security systems etc.).
- May be required to transport staff and/or clients of the Agency and therefore you are to hold a valid Ontario G or G2 drivers license, have a reliable means of transportation and appropriate insurance for work purposes on your vehicle with a minimum of $1,000,000 in liability coverage.
- Willingness to work days/evenings/overnight telephone support (including weekends & statutory holidays)
Salary scale: $29.26 to $38.32 per hour commensurate with qualifications and experience
