Grandview Medical Centre Family Health Team is seeking a permanent, full-time RN or RPN Diabetes Educator.
Our Diabetes Educators work as a team providing support, guidance and education to patients within the Family Health Team.
The main responsibilities for this position are:
- Provide care and education to patients with diabetes, applying best-practice standards that are aligned with professional governing body
- Provide in-depth assessments to patients with diabetes to establish and identify the priorities for the patients care
- Work with patients to develop a care plan that encourages and supports self-management, and that sets goals to achieve desired health outcomes
- Develop and deliver education pertaining to Diabetes that align with patients individual learning styles, and with the patients readiness to move forward in the self-management of their care
- Triage referrals that are received by the Family Health Team to prioritize their level of urgency
- Conduct follow-up appointments with patients, either in person or by telephone
- Ensure quality and integrity of all patient information through the accurate use of electronic medical record (EMR)
- Provide detailed clinical communication to patients family physician through the EMR
- Continually develop educational resources and self-management tools that are aligned with best-practice standards, and that will enhance and support the patients learning objectives
- Collaborate with community partners to provide seamless transfer of information between the acute and community sectors
- Provide education and guidance to other GMCFHT team members pertaining to diabetes
- Collect data as required for the purpose of statistical reporting to Ontario Health
Education, Qualification and Skills
Certified Diabetes Educator preferred
A minimum of 3 years experience working in diabetes care and management
Previous experience working as a diabetes educator with demonstrated leadership, program planning and networking skills preferred
Strong understanding of current clinical practice guidelines related to diabetes
Ability to synthesize and present a comprehensive cardiovascular risk assessment using the Framingham 10-year CVD risk assessment tool for patients.
Ability to assess, monitor and evaluate individual patient needs and develop diabetes management plans to ensure successful outcomes in a patient-centered approach
Ability to develop, implement and deliver health education group programs
Knowledge of pharmacotherapy for dyslipidemia, hypertension and type 2 diabetes
Extensive experience in insulin initiation, titration and pattern management skills involving basal, premix or MDI regimes
Knowledge of insulin-to-carb ratios and carb counting
Must be self-directed and proficient in chronic illness self-management, patient education and motivational interviewing techniques
Must be able to work effectively with a team of integrated health care professionals to provide collaborative team-based care
Comfort delivering education/pt care in person or virtually
Participate in individual or group appointments
Strong working knowledge of Microsoft Office including Word, Excel and Outlook
Excellent communication and organizational skills
Excellent oral and written skills

- Current and ongoing membership in good standing with the applicable college or regulatory body and proof of liability insurance
- Criminal Records or Vulnerable Sector screening
- Proof of COVID vaccination (2-dose series)
