Service Coordinator (Permanent Full Time)


About Us

Traverse Independence primarily supports individuals with acquired brain injuries (ABI) to live independently in the community. As a centre of excellence, we empower and connect clients on their journey to independence. We offer a comprehensive range of services across the continuum of care, including:

  • Adult day programs
  • Services for individuals experiencing homelessness
  • Mainstream outreach in clients' homes
  • Transitional living programs
  • Supportive housing

** This position will have a focus on our adult day program and drop-in centre and as such would benefit from a candidate with a recreation and/or leisure background.


Job Overview

Reporting to the Program Manager, the Service Coordinator will be responsible to:

  • Directly supervise front-line staff through coaching, team building, performance reviews, and performance management.
  • Oversee and evaluate operations through data collection and analysis.
  • Elevate high-risk priority incidents or longstanding concerns that are not resolved.
  • Follow policies, procedures, and processes diligently.
  • Capably use a fully integrated SharePoint and O365 environment.
  • Support the health and safety of both staff and clients.
  • Ensure eligible clients have current assessments, documentation, and service plans.
  • Ensure integration with other sectors and workers involved in the client care team to support wrap-around services for clients.
  • Maintain a professional, collaborative, and supportive culture within the program.
  • Work within an ethical framework, risk management system, and quality improvement philosophy.
  • Have a clear and dedicated understanding of privacy and confidentiality.

Requirements:

  • Community College Diploma in Recreation/Leisure, Social Services, Healthcare, or other related discipline
  • A minimum of 2 years' experience in social services or healthcare with at least 1 year supervising or leading a team
  • Experience in the mental health and/or addiction is an asset
  • Demonstrated experience in behaviour management programs
  • Comprehensive knowledge and experience working in a SharePoint environment, including Microsoft 365 Teams (GoldCare experience a benefit, but not necessary)
  • Problem solving and conflict resolution skills
  • Excellent communication skills (written and oral)
  • Experience working with persons with disabilities and/or acquired brain injuries
  • Understanding of community resources along with issues that relate to substance use, mental health, and brain injury
  • Familiarity with health and safety requirements, AODA and WHMIS
  • Must have valid Class G driver's license and reliable vehicle. Travel will be required.
