The Kee Tas Kee Now Tribal Council is comprised of the five member Nations and is responsible for providing guidance and support to the individual member First Nations in developing and achieving success in the management and administration of their own programs with the intent of developing self-reliance.

The Kee Tas Kee Now Tribal Council is comprised of the member Nations of; Loon River First Nation #476, Lubicon Lake Band #453, Peerless Trout First Nation #478, Whitefish Lake First Nation #459 and Woodland Cree First Nation #474. It was formed in 1995 to facilitate joint action by the member Nations on matters of mutual concern.

Job Description:

Kee Tas Kee Now Tribal Council (KTC) is seeking a dedicated and compassionate Patient Navigator to support KTC Nation members residing in Loon River First Nation, Woodland Cree Nation, and Peerless Trout First Nation. The Patient Navigator will assist Nation members in accessing and navigating the primary health care system, ensuring patients experiences are positive and supported.

This role aims to provide timely, culturally safe healthcare navigation, advocacy, and support for our members.

Responsibilities:

  • Provide navigation, advocacy, and support to KTC Nation members residing in Loon River First Nation, Woodland Cree Nation, and Peerless Trout First Nation.
  • Assist Nation members in organizing and coordinating access to care services; making connections to providers within the patients circle of care, including individualized planning and discharge planning, and providing logistical support when needed.
  • Connect members who are unattached to primary care with available family doctors, social service agencies, and specialty health services (physiotherapy, psychology, homecare, etc.).
  • Advocate for patients' rights and report any incidents of discrimination or racism to KTC and Alberta Health Services (AHS) leadership.
  • Collaborate closely with AHS to ensure effective reporting and resolution of racism and discrimination in healthcare settings.
  • Develop resource guides and communication materials to promote Patient Navigator and primary care services within the member Nations.
  • Support and promote health literacy and health education in KTC member Nations.
  • Monitor client interactions and advocate for necessary resources to address service gaps.

Skills and Abilities:

  • Effective verbal and written communication skills.
  • Proficiency using computers, including Microsoft 360 and other applicable software.
  • Knowledge of the federal and provincial health systems in which First Nations access care, health policies, and Alberta Health Services.
  • Awareness of the current, historical, and intergenerational trauma experienced by First Nations people.
  • Understanding of trauma-informed care principles and practices
  • Ability to work independently and as a part of a team with other care providers.
  • Willing, capable and flexible in working to meet the needs of the health department.
  • Demonstrated knowledge and understanding of Cree culture, organizations and communities.
  • Demonstrated knowledge and understanding of current Indigenous health care concerns and issues.

Qualifications:

  • Related experience in healthcare and advocacy.
  • Excellent communication, interpersonal, and conflict resolution skills.
  • Ability to maintain confidentiality and handle sensitive information with integrity.
  • Strong organizational and time-management skills.
  • Ability to speak and/or understand the Cree language
  • Training from Elders and/or knowledge holders
  • Education in advocacy, social work, health promotion, or other health-related fields
  • ASSIST, Mental Health First Aid, Psychological First Aid, Trauma- Informed Care training
  • Lived experience with health advocacy (individually or with a loved one)

Conditions of Employment:

RCMP/Police Criminal Record Check with Vulnerable Sector and Child Welfare Intervention Record Check (formerly Child Welfare Check, CWIS) submitted prior to commencement of employment.

This job works within our Northern Communities, candidates should be willing to travel in and around the community. Living near or in community would be preferred.

Hours of work are 8:30 4:30pm
