Job Type: TEMPORARY FULL TIME (up to 14 months)
Job Outline:
The Pharmacist will be responsible for providing medication distribution, drug information and education services to patients and staff in a traditional dispensing system. You will be involved in interacting with other interprofessional team members and patients to determine the most appropriate and effective course of treatment for patients and implement interventions in accordance with the standards of practice for the discipline.
Responsibilities:
- Provision of drug information to healthcare professionals
- Support for the Pharmaceuticals and Therapeutics Committee, as delegated by the Manager
- Maintenance of the drug formulary
- Assisting with the management of drug shortages, including identification of alternatives treatments, developing protocols for restrictive use, and communication of selected alternatives and shortages to SHS
- Participate in the development and maintenance of medication use policies
- Participate in educational services, and supporting pharmacy learners
- Provide an orientation to Drug Information services to new staff and trainees
- Assist the Information services team with the development of medication content details to ensure clarity, integration and safety for the Computerized Practitioner Order Entry systems, the Pharmacy system and the electronic MAR
- Support Pharmacy department quality improvement and medication management audits
- Other duties as assigned by the Manager
Education & Qualifications:
- Baccalaureate degree in Pharmacy
- Part A License to practice with the Ontario College of Pharmacists
- Previous related experience in a hospital setting is required
- Excellent oral and written English communication skills
- Proficiency in using Microsoft Office Suite
- Previous experience with Meditech is an asset
- Membership with the Canadian Society of Hospital Pharmacists (CSHP) is preferred
- Demonstrated satisfactory job performance and attendance history required
