North East Regional Virtual Care Clinic System Navigator

Location: Primarily Remote with flexibility to travel to any Maamwesying location as required. Residents of Ontario Health North East Region are strongly preferred.

Employment Status: Full-Time 1.0 Temporary Contract until March 31, 2025, with the potential for extension.

Hours of Work: Up to 37.5 hours per week. The Clinic runs daily from 1pm-9pm, 7 days a week.

Maamwesying North Shore Community Health Services in collaboration with the East Region Virtual Care Clinic, is leading the development and implementation of the North East Region Virtual Care Clinic to facilitate enhancing access to episodic primary care services for all North Eastern residents.

The opportunity: The North East Region Virtual Care (NERVCC) Clinic is a free, Nurse Practitioner (NP)-led virtual walk-in-clinic that serves clients who are experiencing an urgent medical condition and need timely access to care throughout the Ontario Health North East Region. This service is being delivered in partnership with the East Region Virtual Care Clinic (ERVCC). The clinic, currently in its development stages, serves clients with or without a primary care provider and with or without a health card.

Under the direction of the Director of Primary Health Care, the North East Regional Virtual Care Clinic System Navigator will provide comprehensive, client-centered support to the Nurse Practitioners, in a virtual environment, focusing on accessibility, health promotion, illness prevention, and continuity of care. This role emphasizes understanding the social determinants of health that impact client well-being. The System Navigator will practice to their full scope as outlined by the College of Nurses of Ontario (CNO) guidelines, emphasizing supportive, preventive, therapeutic, and rehabilitative care. The System Navigator will facilitate NERVCC clients with triage, connection to local services, navigation, and referrals. The System Navigator will collaborate with an inter-professional team and will demonstrate leadership in championing program goals and values.

What you need to bring with you:

  • Nursing Degree Bachelor Science in Nursing (BScN)
  • Current member in good standing with the College of Nurses of Ontario (CNO)
  • Minimum of 5 years experience in primary care and/or virtual care settings
  • Experience with Oscar Pro, Ocean Referral and other provincial eHealth assets/systems is preferred
  • Computer proficiency is required
  • Resident of North East Region preferred
  • Excellent interpersonal, communication, assessment, problem solving, and decision making skills
  • Ability to convey information to clients effectively in a virtual setting
  • Experience in supporting the resolution of client concerns
  • Established ability to accurately complete required documentation, reports, and forms
  • Demonstrated ability to work independently in a virtual environment while collaborating with an inter-professional team
  • Comfortable working in a developing and evolving program
  • Knowledge of the health care delivery system and assist clients with navigating community resources
  • Effective time management, prioritization, and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary virtual environment
  • Experience working with Indigenous communities is an asset
  • Experience working with the Francophone population is an asset

What to expect? Given the traditional practices of Indigenous people, from time-to-time exposure to smoke from the burning of sacred medicines such as tobacco, sweet grass, sage, or cedar may occur.
