Based within the courthouses we serve, our YCAPP team are there to help young people and their families as they navigate the youth justice system. We understand that the process is overwhelming and are there as a support in navigation. We work with the young person to create an action plan, which is designed to support them to achieve their immediate and medium-term goals. We also work together to identify and connect the youth/their families to needed community-based resources, based on the individual's needs.

Reporting to the Supervisor, the Youth Court Action Planning Program (YCAPP) Coordinator is responsible for supporting the YCAPP staff, placement students, and volunteers in working in collaboration with the young person to develop a plan for community-based interventions that respond to youth strengths and needs and seek to repair harm and reconnect the young person to their community. This role is also responsible for case coordination of referred youth, as well as providing coverage at the various sites as needed.

Team Leadership

  • Work closely with leadership within the youth community justice programs, to provide support and guidance in the delivery of the program to the Youth Court Action Planning Program team, including staff, placement students, and volunteers
  • Support in the recruitment and oversight of placement students and volunteers
  • Ensure that programming and operations are in accordance with the program model, the program vision and mission, and requirements of service agreement/funder

Program Delivery

  • Establish relationships with youth and their families and/or supports to support their ongoing engagement and involvement in their action plan
  • Support the staff team in implementing the program model within the daily operations and programming and provide feedback and coaching as necessary
  • Modeling and supporting the staff team in the use of crisis prevention, intervention, and other de-escalation strategies for clients who have experienced trauma
  • Oversee the daily activities of the program, including ensuring that regular programming and supports are in place for the youth
  • With leadership, support in ongoing evaluation and feedback meetings with youth and staff to ensure youth and staff voice is represented in all facets of the program
  • Support in the development, collection, and review of evaluation data to support in ongoing program fidelity and growth

Partnerships

  • Develop and strengthen community partnerships and referral sources for clients, community organizations, mental health supports, youth shelters, etc.
  • Work closely with referral sources and partners including court personnel (such as Crown Attorneys, defense counsel, Probation, etc.), Toronto District School Board (TDSB), Toronto Police Service (TPS), community organizations, etc.
  • Foster relationships and represent the YCAP program and organization at network tables, community groups, conferences, etc. as assigned

Administration & Professional Development

  • Maintain timely and accurate notes and logs using case management and other software, as well as other administrative tasks, as per our program model and policies
  • Regular check-ins with YCAPP team to ensure that program files are up to date and to ensure timely and accurate reporting of referrals, case notes, meeting minutes, and other administrative duties
  • Support in the reporting of statistics and other notes as needed, to support in the program evaluation or other special projects, as assigned
  • Actively participate in individual and team meetings, supervisory meetings, planning sessions, case management meetings, and designated trainings
  • Work effectively with other members of the team
  • Modeling the agency's philosophy and mission statement, as well as the program's Values and Beliefs at all times
  • Other responsibilities as may be assigned
  • Administrative Support - Gather, input, maintain and correlate statistical data. Prepare and submit all required program reports and documentation. Provide training and coaching to students or volunteers. Represent the agency in workshops, conferences and committee meetings. Deal with crisis situations that may involve volatile behavior.

WHO YOU ARE

  • Minimum diploma in Social Work, Child & Youth Work, or relatedfield and a minimum 2 years related experience or equivalent.
  • Knowledge and some experience working within the youth justice system
  • Strong understanding of the many community organizations and supports available to young people
  • Direct experience engaging with diverse populations and an ability to apply a culturally responsive lens to all interactions, including with clients, staff, and partners
  • Excellent interpersonal, written, and verbal communication skills and a demonstrated ability to collaborate with multiple stakeholders effectively, respectfully, and efficiently with minimal supervision
  • Proven ability to prioritize shifting responsibilities, strong time management skills, and leadership abilities
  • Proven ability to work collaboratively within and across teams
  • Satisfactory Vulnerable Sector Screening

In addition, the following are considered assets:

  • Understanding of relevant legislation and institutional policies (including the Youth Criminal Justice Act, Education Act, Safe Schools Act)
  • Knowledge of the social service agencies operating in Toronto and a willingness to broaden knowledge with respect to clients coming from outside of our geographic location
  • Current First Aid/CPR certification
  • Non-Violent Crisis Intervention or Crisis Prevention and Intervention certification
  • Applied Suicide Intervention Skills Training (ASIST)
