Position Summary
Reporting to the Manager, Community Programs, the Community Transportation Coordinator is responsible for the overall coordination and administration of the Community Transportation Program. This includes arranging automobile or van transportation for clients to and from appointments and medical facilities, collaborating with partnering agencies and internal departments, providing supervision and care for clients while transporting them, coordinate both drivers and the safe, efficient operation of DHNS fleet vehicles for client trips in the City of Toronto, and create a safe and supportive environment for all participants and staff in the department. This position works closely with the Meals on Wheels Supervisor who directly supervises the Community Transportation drivers.
Duties and Responsibilities
Program
  • Conduct in-home assessments to determine eligibility for the Community Transportation Program.
  • Coordinate daily scheduling of rides using Trapeze Scheduling System.
  • Refer clients to other programs and services where appropriate.
  • Liaise with Toronto Ride partners.
  • Participate in Toronto Ride Coordinator's meetings.
  • Maintain and coordinate client information and program statistics through CIMS program.
  • Respond to client inquiries and complaints.
  • Train, schedule and direct Community Transportation staff and volunteer drivers
  • Respond, coordinate and follow up with Home At Last Referrals.
  • Ensure safe and secure operation of vehicles in compliance with the Highway Traffic Act.
  • Attend meetings as required and liaise with other agencies, organizations and professionals in the community.
  • Assist with other programs as necessary.
Organizational Accountability
  • Work collaboratively with seniors department staff, other Dixon Hall staff and departments and volunteers.
  • Operate in an effective, efficient and client focused manner to achieve the best outcomes for clients.
  • Actively participate in department and agency meetings and events.
  • Work under the guidance of the agency and program policies and procedures, including the Mission, Vision and Values of Dixon Hall and the agency's strategic direction.
  • Meet regularly with manager to discuss individual, team and the overall effectiveness of client services.
  • Undertake other tasks and responsibilities as assigned within the purview of the job description.
  • Other duties as assigned.

Position Qualifications
  • University degree or college diploma in a relevant field.
  • 3-5 years' experience working with seniors and adults with a disability or illness.
  • Excellent organizational, interpersonal and communication skills.
  • Strong verbal and written English language skills; the ability to speak other languages will be considered an asset.
  • Skilled communicator who has demonstrated effectiveness imparting information across multiple communication barriers.
  • Able to work effectively in a team setting.
  • G class driver's license; qualification at higher levels will be considered an asset.
  • Proficient with Microsoft Office (Word, Excel, Outlook, Publisher) and database management.
  • Must be qualified to work in Canada legally for the employer.
  • Able to provide a Vulnerable Sector Screening check with a ‘Clear' or satisfactory result that meets the operational requirements of the Agency.
  • Must have up-to-date vaccinations against COVID-19 with a vaccine that has been approved for use in Canada.
