Job SummaryThis is a casual position with the possibility of part-time. Registered Practical Nurse is a member of the home care team who works under the supervision of the Registered Nurse and performs various tasks based on the client's plan of care
Client Care Management
  • Performs assessments in the community ( including nursing)
  • Ensures standards of care, compliance with College of Nurses standards, state/provincial regulations and standards of care
  • Develops standards that ensure safe and effective services to clients and families;
  • Establishes care plan, completes and maintains related records and documentation
  • Ensures a high standard of customer service is delivered to clients from intake to client conversion, case setup- up and delivery of service;
  • Ensures that the level of care established for clients is safe and appropriate based on client and home assessment
  • Directs the implementation of service goals and objectives
  • Ensures the evaluation of client care services
  • Achieves/exceeds client satisfaction, investigates issues, and follows up promptly;
Qualifications
  • Registered with the College of Nurses of Ontario (CNO) in good standing
  • Experience in doing assessments in Home and Community Care ( 1-2) years
  • Current BCLS certification (Level C)
  • Demonstrated clinical assessment and patient/family teaching skills
  • Proven excellent interpersonal skills and effective oral/written communication skills
  • Demonstrated excellent organizational, time management, prioritization problem-solving, and conflict-resolution skills
  • Demonstrated planning and coordination abilities
  • Proven ability to work effectively as part of a team
  • Demonstrated ability to work with minimal supervision
Requirements:
  • Clear & updated Vulnerable Sector Check (within the last six months)
  • Valid Drivers License & reliable car
  • Lives near or willing to travel to Runnymede and Bloor
  • Up-to-date TB test within the last four years
  • High level of customer service experience
  • Proven interpersonal, organizational, written, and verbal communication skills.
  • Ability to work independently in a fast-paced, diverse, and multicultural environment.
  • Demonstrated commitment to providing the highest level of satisfaction in client and staff service care
Schedule:
  • Ability to see clients when needed. The ideal candidate is someone who has a part-time job and is able or willing to work casually
