JOB SUMMARY
Reporting to the Care Manager, the Resident LPN Manager (RLM) provides leadership, support, direction and supervision to the front-line nursing staff on an ongoing basis. The RLM plays a key role in taking the lead in the Resident's assessment, planning, implementation, and evaluation of care according to the Resident's individualized Care Plan. The RLM is responsible for facilitating the quality improvement in Resident care by evaluating and monitoring the processes in the care units. The RLM accepts responsibility for continuing competencies through ongoing professional development including participation in education programs, research and continuous quality improvement activities and works within the scope of practice as defined by the College of Licensed Practical Nurses of Alberta (CLPNA).

DUTIES AND RESPONSIBILITIES
  1. Ensures the ongoing monitoring of care is delivered according to the Care Plan. Collaborates with Care Managers and AHS Case Managers with the implementation and evaluation.
  2. Develops structures and processes which integrate evidence-informed nursing practice by analyzing, interpreting and using information to support decisions in the practice environment.
  3. Assists in planning, developing, implementing and monitoring site quality improvement and risk management processes as per Park Place Seniors Quality Improvement Framework.
  4. Identify and define barriers to quality resident care and provide innovative and collaborative solutions.
  5. Role-models best practices and collaborates within the nursing profession to enhance Resident care.
  6. Works with the Leadership Team, Care Staff, AHS professional team and physicians to discuss current initiatives and resolve issues as related to best practice and improved care delivery.
  7. Completes internal audits to ensure Standards and Park Place Seniors policies and processes are met.
  8. Recruits, leads and supervises the interdisciplinary Team to ensure effective, consistent, and comprehensive resident care planning and delivery based on Residents' choice and PPSL Policies and Procedures in collaboration with the Care Manager.
  9. Provides leadership and support through a respectful, collaborative approach to monitor, supervise and direct the interdisciplinary team in care provision. Assists with new staff orientation and routine performance appraisals as allocated. Offers staff mentoring, education and/or discipline as required up to and including termination.
  10. Carry the on-call phone on a rotational basis to assist and support the Care staff with emergent/urgent situations.
  11. Establishes and maintains therapeutic relationships with Alberta Health Services, families, volunteers and others through effective interpersonal and interviewing techniques.
  12. Performs other related duties as assigned.
Program Development
  1. Is responsible for the development, implementation and ongoing operation of the community.
  2. Works in collaboration with the interdisciplinary team to promote health and safety awareness, safe work practices and monitors nursing staff for safe, competent professional practice.
  3. Assist with various care programs as needed e.g. Wound care, flu clinics, monthly audits, new projects as allocated by the Director of Care.
  4. Commitment to resident safety and to our “culture of safety” as per PPSL Strategic Directions and Home Goals.
  5. Identifies and acts on the need to change and improve the clinical environment. Develops and monitors implementation plans for change in collaboration with the Care Manager. Explains the need for change and growth in terms of community goals – assists staff in understanding and coping with change and focuses on the implementation of changes in a manner that maintains and improves community service levels.
Collaboration with Clinical Team
  1. Facilitates staff participation and engagement in problem solving and develops a positive work environment.
  2. Reviews care plans on admission and as needed with the care staff.
  3. Sets high standards for personal and professional conduct of the nursing department.
  4. Takes lead on PAVs, Admission, and Discharge processes, doctor rounds, and case conferences.
  5. Implement care plans and ensure that care staff are aware of any changes.
  6. Monitor resident issues and provide support to staff if needed.
  7. Attends shift report and discussion with the Case Manager.
  8. Liaison with physicians and other members of the healthcare team.
  9. Provides assistance in the community when staffing shortages.
  10. Participates in assessments, by reviewing the resident's chart and collaborating with the interdisciplinary team to ensure person-centered care using the Resident Day.
QUALIFICATIONS
  • Active Registration with C.L.P.N.A.
  • Current CPR and First aid required.
  • Clear criminal record security clearance with Vulnerable Sector included (no more than 6 months old)
  • General computer skills, strong written and verbal communication skills.
  • Position requires flexibility in working hours in order to meet the needs of the staff.
  • Ability to organize and prioritize a changing environment.
  • Must be able to work independently, and as part of the interdisciplinary team

  • Teamwork and Collaboration: Demonstrates ability to work together with others toward shared goals and desired outcomes.
  • Professionalism: Demonstrates ability to effectively establish professional working relationships with residents and their families as well as members of the care team aiming for excellence in residents' care.
  • Communication: Demonstrates ability to communicate effectively with residents, families, the public, medical staff and other members of the interdisciplinary team. Ability to intervene effectively to resolve conflict and manage stress and work pressure.
  • Results Oriented: Ability to apply continuous quality improvement tools and processes within a dynamic environment, leading to improved care and service delivery.
  • Equipment: Demonstrates applicable computer skills and knowledge of nursing-related care equipment utilization and management.
SALARY - To Be Discussed
PHYSICAL DEMAND
Job requires a significant amount of lifting, bending, walking, sitting and standing. Lifting 50-lbs. maximum. More frequent lifting of 20 lbs. or less. A regular day may involve four (4) hours of walking and frequent bending or crouching. Must be willing to work flexible hours.
