Location: Regional – Remote Work Enabled

Term: 1.00 FTE Temporary

About the Organization:

The Barrie Area Native Advisory Circle is a regional social planning and development organization. The BANAC service region now covers Simcoe County, Muskoka, and the northern York Region. BANAC works in cooperation with communities in areas of concern where coordinated efforts are required to meet the needs which positively impact the region. BANAC will continue in this social planning capacity, offering consultation and support for our community development. BANAC offers many innovative programs and services including Alternative Dispute Resolution, Family Wellness, Childcare, Seniors Care and Long-Term Care, Cultural Services, and wholistic Health Services through the Mamaway Wiidokdaadwin Indigenous Interprofessional Primary Care Team.

About the Role:

Anishinaabe Miikan, Red Road to Recovery Program is an Indigenous Guided Mental Health and Addictions Program. Partnering with local indigenous organizations, we are able to provide a wider range of supports for those in need. Red Road to Recovery looks at the physical, psychological, and spiritual aspects of addiction – while addressing the individual needs of participants.

As a part of the interprofessional primary care team, the Red Road to Recovery Program, Case Manager's role is to support the development of a comprehensive supportive treatment plan, coordinate physical, emotional, mental, spiritual, and social health services, and to make appropriate referrals and linkages to community services and resources. The Red Road to Recovery Program, Case Manager provides services in a supportive, confidential, and culturally safe manner to help clients work towards their goals and access the resources they need to fully participate in health care services. The Red Road to Recovery Program, Case Manager is responsible for case management specific screening, assessing, planning, connecting, and transitioning of care. The care provided by the Red Road to Recovery Program, Case Manager is time limited, and goal focused. The Red to Recovery Road Program, Case Manager will participate in rounds, team meetings, care conferences, and client appointments as necessary to support the fulfilment of their case management plan. Although this role will work closely with therapists and social workers, the Red Road to Recovery Program, Case Manager does not provide therapy or counselling to the clients they support. The Red Road to Recovery Program, Case Manager will receive care guidance from Addictions and CWS Team members in the services they provide. Clients will be connected through the CWS program, Addictions related programs as well as other areas of Mamaway Wiidokdaadwin as assigned.

What You Bring:

  • Completion of a Social Service Worker Program and 2 years' experience; or an equivalent combination of education and experience.
  • Completion of training in case management is considered an asset.
  • Demonstrated experience working with Indigenous communities and clients; ability to provide culturally safe care
  • Experience providing case management services to clients with complex needs in the social determinants of health, particularly homelessness, mental health and addictions, and transportation.
  • Experience in implementing risk management and organizational protocols
  • Previous experience working within Indigenous communities or organizations.
  • Ability to provide Indigenous culturally safe, trauma-informed care and services.
  • Ability to understand Indigenous cultural values, traditions, history.
  • Ability to integrate Indigenous perspectives into services.
  • Ability to respect individuals' experiences, foster healing.
  • Maintain a time-limited and goals focused case load from internal referrals sent by Mamaway Wiidokdaadwin clinicians and team members
  • Screen clients for eligibility in case management through MWIIPCT
  • Perform a comprehensive assessment of a client's social and community needs as they relate to a case management service
  • Develop a collaborative Case Management Plan
  • Facilitate coordination, communication and collaboration with clients and internal and external care partners
  • Develop rapport, trust and ethical relationships with clients and stakeholders.
  • Collect and synthesize relevant information and perspectives from clients, social networks, and internal and external care partners where applicable.

Information About Working at BANAC Mamaway Wiidokdaadwin:

  • Work is typically performed in clinics, office, institutional and community settings.
  • Regular travel to meetings, institutions, and communities will be required – vehicle an asset.
  • Regular overtime work may be required from time to time, BANAC team members enjoy a flex schedule agreement.
  • Periodic exposure to smoke may occur from the burning of sacred medicines: Tobacco, sweet grass, sage, or cedar.

Specific employment conditions are specified in an employment contract with BANAC and include the following:

  • Worksite location
  • Valid C.P.R. and A.E.D. Certificates required.
  • Valid Class G Ontario Driver's License is required with clear check for vulnerable sector.
