Working as part of an interdisciplinary team to provide clinical health services for clients and members of our priority populations, the Nurse Practitioner (NP) provides specialized health care with a strong emphasis on health promotion and disease prevention within the scope of practice and standards of care outlined in the Expanded Nursing Services for Patients Act and Regulations and the Practice Expectations Specific to Registered Nurses in the Extended Class (NPs). The NP diagnoses and manages common health problems and diseases both independently and in collaboration with the client and the physician. The Nurse Practitioner works as part of an interdisciplinary team to address issues in the community that affect health. They are responsible and accountable for their practice.

Main Responsibilities

  • Provide ongoing, comprehensive health care to clients within priority population group, including addressing the health care needs of clients. This may include comprehensive assessments, diagnosis, counselling, screening, referral, education, treatment and follow-up, supportive client counseling, and crisis intervention.
  • Determine the presence of existing and potential health problems, with a major focus on lifestyle, psychological, sociocultural and environmental risk factors that may influence the client's health status.
  • Ensure appropriate, comprehensive treatment is delivered and maintain complete, accurate, and timely electronic medical records.
  • Participate in chart reviews, discussion of problem cases, and client inquiries
  • Refer to specialists, hospitals, or other agencies as required
  • Work within a determinants of health framework which emphasizes health promotion and education, disease prevention and recognizes the special risk factors (social, physical, psychological etc.) which may affect the health of clients
  • Assure standards of patient care are maintained in accordance with accepted current best practice guidelines & Guelph CHC policy.
  • Maintains client confidentiality and acts in an ethical and professional manner in accordance with the codes of conduct prescribed by the College of Nurses and in accordance with the Centre's policies.
  • Participates in other GCHC programs/projects, as appropriate.

Key Qualifications and Skills

  • Registration in the Extended Class with the College of Nurses of Ontario
  • Member of the Nurse Practitioners Association of Ontario
  • Experience working with communities and individuals who face barriers to accessing health care due illness and other factors known to be the determinants of health (poverty, education, language barriers, discriminations, mental health and substance use issues, etc.).
  • Previous experience in community health/social services and/or a values-based organization is an asset
  • Provides education and client centered care in a respectful and supportive manner,
  • Able to independently manage complex clients with comorbidities and vulnerabilities, consulting as appropriate
  • Excellent communications skills, both verbal and written, along with the ability to prioritize and multi-task.
  • Respectful, empathic listening with explanations geared to the clients abilities to understand.
  • Experience working with cultural interpreters an asset.
  • Proficient in the use of computers and various other software applications including Word, Outlook, PSS and electronic health records
