At Stay at Home Nursing, we are looking for a Care Manager who will help coordinate and oversee personalized healthcare at home, ensuring that our clients receive the highest quality care tailored to their needs.

As a Care Manager, you'll deliver exceptional in-home care, build strong client relationships, and collaborate with a multidisciplinary team to ensure positive outcomes. This role combines in-office, hybrid, and community fieldwork.

Key Responsibilities:

• Create and update personalized Care Assessments.
• Conduct supervisory visits and ensure quality care.
• Educate clients and adapt care based on risk issues.
• Lead and support caregiving teams.
• Collaborate with Scheduling to manage client coverage.
• Conduct Caring Consults and aim for an 80% conversion goal.
• Maintain accurate documentation and ensure compliance.

Requirements:

• Bachelor's Degree or Nursing Diploma.
• RPN certificate and CNO license.
• 1-2 years of nursing experience preferred.
• Valid driver's license and car.
• Proficient in G-Suite and Microsoft Office.

Work Schedule: Full-time, Monday to Friday, with on-call as needed.

Salary: $60,000 - $66,000 per year
