Position Summary
Reporting to the Supervisor, Assisted Living & Home Support Programs (Respite Care and Homemaking Programs), the Client Support Coordinator is responsible for scheduling and coordinating personal support workers and home support workers according to client care needs. The position will be responsible for ongoing assessment and adjustment of the provision of care and service delivery for seniors supported in Assisted Living and Home Support Programs.
Duties and Responsibilities
  • Monitoring and creation of schedule with approval and confirmation from supervisor of programs.
  • Ensure clients are set up with required care and services in a timely and coordinated manner.
  • In collaboration with the client, substitute decision-maker, caregiver and/or other service providers develop, evaluate and revise individual client care plans.
  • Conduct in-home assessments to assess client needs.
  • Maintain professional, ongoing communication with Ontario Health at Home, primary health care professionals, clients and community partners involved in a client's care.
  • Resolve or escalate, as required, client, family or staff concerns or issues.
  • Be available to support 24-7 program when required.
  • Handle incoming calls, emails, and correspondence, ensuring timely and professional responses.
  • Maintain accurate and up-to-date client records and documentation in client database.
  • Ensure client scheduling records are up to date on for monthly invoicing and statistical reporting
  • Assist in preparing reports, presentations, and other materials as required.
  • Collaborate with program supervisor to collect relevant data and maintain database.
  • Assist with basic financial tasks, such as tracking expenses, collecting, receipting and submitting client fees, and maintaining client petty cash.
  • Support in the onboarding process for new staff.
  • Maintain a clean and organized office space.
Qualifications
  • Two to three years experience working with scheduling, client care and/or supporting older adults.
  • Bachelor's degree/College Diploma in a relevant field (e.g., Gerontology, Health, Social Services, Social Work).
  • Comprehensive knowledge of senior issues and the continuum of care.
  • Strong communication, interpersonal and organizational skills.
  • Strong assessment skills with knowledge of InterRAI Assessment an asset.
  • Demonstrated planning and coordinating abilities.
  • Collaborative and able to work effectively within a team or independently in a fast-paced working environment.
  • Ability to exercise sound judgment.
  • Excellent computer skills, including proficiency in using Office 365 (e.g., Outlook, Word, Excel, PowerPoint, Teams, SharePoint, etc.).
  • Experience with AlayaCare an asset.
  • Must be legally able to work in Canada.
  • Able to provide a Vulnerable Sector Screening check with a 'Clear' or satisfactory result that meets the Agency's operational requirements.
  • Must have up-to-date vaccinations for COVID-19 with a vaccine that has been approved for use in Canada.
