We are seeking an experienced and passionate Care Manager to join our office team and play a pivotal role in a growing home care delivery business!

JOB DESCRIPTION SUMMARY

The Care Manager is responsible for supporting the Director of Clinical Services in managing our delivery of Home Care services from our office in NW Calgary. They work with our caregivers, clients and their families to allow our clients to remain at home with the highest standard of living possible. The Care Manager also aids in the scheduling and recruitment of Caregivers.
DUTIES & RESPONSIBILITIES
Client Care
  • Works directly with clients and their families in clients spaces as well as from our central office;
  • Visits clients on a regular basis;
  • Introduces caregivers to clients, their family and their home, and familiarizes caregivers with the Care Plans and adjustments to Care Plans;
  • Acts as first point of contact for clients requesting care, through phone calls and email enquiries as needed;
  • Performs case management for clients, including liaising with doctors, pharmacy and AHS medical personnel;
  • Responds to client and family requests and concerns;
  • Creates and facilitates effective and efficient client care documentation through our mobile app.
Caregiver Management
  • Responds to caregiver requests and concerns, and acts as an advocate on their behalf;
  • Works closely with Human Resources to ensure that clients are properly scheduled in a client-centred and efficient way. The Care Manager may be required to cover for absent Caregivers, Monday to Friday, in urgent circumstances;
  • Participates in Caregiver selection processes including recruitment, hiring and onboarding of staff when required;
  • Collaborates with management on staff development including caregiver orientation, in-service education, continuing education, and evaluation of staff;
  • Supports the Director of Clinical Services in administrative and client care policies and practices, while maintaining Qualicares standards of service.
The Care Manager is required to be on an ON CALL rotation with the management team, from Monday to Friday.
The Care Manager must be willing to travel to clients homes, as frequently as daily, as per the needs of the business.
