Position Summary:

Alberta Indigenous Virtual Care Clinic (AIVCC) seeks two part-time Registered Nurses for compassionate support of Indigenous patients across Alberta. As a member of our team, you will work alongside healthcare providers, families, and other stakeholders to ensure a culturally safe patient care experience. The Registered Nurse is responsible for virtual care directly with patients including assessments, health histories, medication reconciliation, cancer screening, health education, and immunization status as per Accreditation Canada required organizational practices. Screening measures will be implemented from the Alberta Screening and Prevention (Asap), and the successful candidate will oversee chart reviews and work collaboratively with the team to ensure patient screening is completed.

Our ideal candidate is a Registered Nurse who is a skilled communicator, helping patients navigate complex healthcare and social systems while identifying their personal and medical needs and acting as an advocate for their health. The successful candidate will have strong collaborative skills, exceptional organizational skills, and be comfortable working with AIVCC physicians, psychologists, and staff, as well as external agencies.

As AIVCC represents a unique model of care, the successful applicant will be responsive to emerging needs of patients and communities with respect to services provided.

The ability to work remotely is a requirement of the position. Funding for this position has been provided through a unique partnership between federal sources.

This position will may include daytime, evenings and weekends and will be a maximum of 18 hours per week.

Responsibilities:

  • Supporting treatment of patient's health conditions
  • Formulating treatment plans with team for acute and chronic diseases
  • Advocating for patient cancer and diabetes screening
  • Educating patients on lifestyle habits, disease processes/management, medications, lab work, testing, procedures and immunizations
  • Reviewing EMR and Netcare for out of date cancer screening, consultations, medications, or lab work or immunizations
  • Ensuring patients are given appropriate resources for social determinants of health – working alongside Patient Navigators of clinic
  • Collecting and documenting demographics, allergies, vaccinations, medications, personal and family health history
  • Providing a safe space for patients and clients to help promote health literacy
  • Thorough and complete documentation of patient care in clinic
  • Promoting patient and client confidentiality and compliance with Personal Health Information Protection Act (PHIPA)
  • Assisting MOAs in triaging of patients
  • Assisting Clinic Manager and Physicians with flow of care in clinic with ever growing patient and client load
  • Identifying and prioritizing gaps in health literacy for patients and clients – able to assist these patients take control of their health care

Requirements:

  • Bachelor of Science in Nursing (BScN)
  • Clinical training or experience with Connect Care, Netcare or Med Access EMR
  • Registered with CARNA
  • Minimum of 3 years' experience with any of the following: Oncology outpatient/inpatient nursing, public health nursing, community health nursing, and clinic nursing
  • Open to guidance and mentorship within the clinic
  • Experience working with Indigenous patients
  • Excellent interpersonal and communication skills
  • Strong computer skills and capable troubleshooting skills
  • Time management and organizational skills
  • Team player and self motivator
