Job Description5C Nephro/Neuro is a 36 bed medicine unit. We have renal dialysis hook-ups in the rooms to allow for patients to have dialysis in their room if needed. Our patients may have acute or chronic renal diagnoses, they may have experienced a stroke or have other neurological diagnoses. We have a multi-disciplinary team including physicians, nurses, physiotherapists, occupational therapists, pharmacists, dietitian and discharge planner who collaborate to provide safe, high quality care in a timely fashion to our patient population. We practice patient and family-centered care.
A Registered Practical Nurse assigned to this unit will primarily be responsible for applying the nursing process to acute medical patients with complex renal and stroke issues including end-stage renal disease, peritoneal dialysis, acute stroke and TIAs. You'll be responsible and accountable for all elements of the nursing process when providing direct patient care including assessing, planning, implementing and evaluating care based on specific components. You'll assume responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, and supporting factors that impact customer satisfaction. You'll administer medications and procedures per established policies and guidelines. You'll also be documenting observations, assessments and changes in patient's condition; collaborating with health team members to facilitate positive patient care outcomes as well as providing a full range of nursing care to patients.

- Current Registration in good standing with the College of Nurses of Ontario.
- Current Heart & Stroke approved Basic Life Support (BLS) certification required at time of hire
- Current acute inpatient medicine experience preferred
- Experience working in a neurology and/or nephrology environment preferred
- Demonstrates commitment to continuing education.
- Required skills include aseptic technique, suctioning, wound care and IV insertion.
- Experience with medication administration required.
- Ability to function in a fast-paced environment.
- Team player with strong public relations and organizational skills.
- Caring, compassionate attitude with patients, families and co-workers.
- Must be able to meet the physical demands of this position.
- Satisfactory employment and attendance record required.
The ideal candidate is strong in patient centred care in a dynamic, rapidly changing environment and should be ready to drive change and innovation. We require a person with good communication skills with a clear patient focus with the ability to manage execution and to foster teamwork and cooperation.
Additional Information
Hours: Monday - Sunday various rotating 12 hour shifts. 0730-1930, 1930-0730. Hours subject to change based on operational requirements.
Salary: $33.26 - $35.97 per hour
