REGISTERED PRACTICAL NURSE – Community Support Services
Transitional Beds Program
POSITION SUMMARY:
The Registered Practical Nurse – Community Support Services, Transitional Beds Program (RPN - TBED) is responsible for providing quality and compassionate nursing care to individuals residing in the First Place Transitional Beds Program The RPN-TBED is accountable for enhancing the life of the residents by providing a range of care with predictable outcomes, within the framework of SJHC's policies and procedures and the standards of practice prescribed by the College of Nurses of Ontario. This role contributes to the healing and overall well-being of the residences, and promotes the continual improvement of the overall health of the Transitional Care Bed Program clients.
The RPN - TBED works to support the Clinical Nurse Manager in the day to day clinical care operations of the program and works collaboratively with other care givers / staff, in order to help ensure the delivery of high-quality client care and effective clinical functionality. The RPN-TBEDS also supports the Manager – Operations, First Place in the day to day operational requirements of the program as necessary or requested.
CORE DUTIES AND RESPONSIBILITIES
Clinical

  • Performs comprehensive and focused health assessments, including health history and complete physical evaluation, current knowledge, education, literacy levels, social supports, learning preferences, and other factors affecting the education approach, plan, and treatment.
  • Uses and adapts assessment tools and techniques based on residents' unique needs and best practices.
  • Utilizes the Nursing Process to assess, plan, implement, and evaluate resident care, in consultation with the Clinical Manager, on an ongoing basis.
  • Reports resident symptoms, assessments, monitoring outcomes, and consults with the primary care physician through SBAR.
  • Participates in the admission process by completing the resident intake and physical assessment tool. Performs subsequent assessment based on necessity, ensuring comprehensive evaluation and tailored care planning for each resident upon admission.
  • Implements and discusses individualized care plans with residents/POAs/SDMs, in collaboration with residents, Clinical Manager, and interdisciplinary team members.
  • Monitors, evaluates, and adjusts care plans based on the effectiveness of interventions and/or changes in condition or environment, in collaboration with residents, Clinical Manager, and interdisciplinary team.
  • Conducts risk assessment in collaboration with residents and other service providers, developing appropriate health plans to promote health and prevent illness and complications.
  • Participates in weekly interdisciplinary meetings to communicate needs/issues concerning assigned clients, ensuring coordinated, comprehensive, and holistic services to achieve client health and discharge goals. Works collaboratively with the team to support health promotion, disease prevention, and chronic disease management.
  • Provides skin and wound care, including dressing changes, irrigation, packing, and changes to colostomy appliances and catheter as ordered.
  • Assists Clinical Manager in implementing and assessing the efficacy of diverse best practice protocols, including but not limited to Continence Care, Bowel Management, Catheter Care, Promotion and Management of Continence, Foot Care, Pain Management, Fall Prevention and Management, Behavior Management, Acute/Medical Emergency response, Palliative Care, End-of-Life Planning and Care, Chronic Disease Management, Supporting Nurse Practitioners in procedures, Delegation, Conducting Focused Nursing Assessments, Admission and Discharge procedures, and Providing Health Education and Counseling.
  • Identifies the complexity of care situations within the scope of practice and utilizes appropriate protocols for resolution.
  • Determines the need for and initiates referrals and consultations with the interdisciplinary team, external service providers, and primary healthcare providers in a timely and supportive manner.
  • Completes referrals, lab requisitions, and diagnostic requisitions as ordered by the primary care physician/NP.
  • Processes prescriptions, transcribes orders, and facilitates medication ordering according to the facility's pharmacy policy.
  • Coordinates services and appointments with residents/SDMs/POAs to ensure continuity and follow-up, informing and educating them regarding the meaning and implications of test results and interventions.
  • Provides daily huddles with personal support workers and unregulated care providers about needs/issues concerning assigned clients, encouraging cooperation between all staff.
  • Documents and reports client information according to SJHC policy, congruent with College of Nursing standards of documentation.
  • Provides high-quality nursing care for clients and/or caregivers in all clinical situations, contributing to the efficient functioning of the program and the attainment of goals.
  • Follows plans of nursing care within the scope of RPN practice, ensuring effective caseload management and performing cost-effective client care through collaboration with management, Clinical Nurse Manager, and other stakeholders.
  • Facilitates client/caregiver problem-solving and resolution, participating in various decision-making frameworks, and assuming leadership/coordination responsibilities as required or requested.
  • Provides education related to healthy living, chronic disease management, prevention of injury, illness and communicable diseases, care and treatment, individual and family adjustments, and support systems as appropriate to the client situation, employing education strategies and motivational interviewing skills to support behavior changes to enhance health (e.g., smoking cessation, physical activity, and healthy eating).

KEY EDUCATION AND EXPERIENCE
  • College level degree in Practical Nursing, required AND
  • Member in good standing with Ontario College of Nurses, required AND
  • Current and unrestricted RPN licence, required
  • Current First Aid with CPR certificate, required
  • Current Basic Cardiac Life Support (BCLS) certificate, required
  • Experience conducting comprehensive nursing assessments, required
  • Gentle Persuasion Approach certification, required
  • 3-5 years of previous RPN experience, required
  • Working knowledge of MS office software including excel spreadsheets, required
  • Previous experience working in a Community Care model or Transitional Bed setting, an asset
