You will be responsible for providing high-quality nursing care to patients in the community. You have clinical skills and extensive knowledge of wound, ostomy and continence care management for a wide range of patients with chronic diseases.
- Provide patient-focused, safe, holistic nursing care and work collaboratively with other members of our team
- Ensure that nursing care provided to our patients is based on current and best practices to achieve successful outcomes
- Continually assess, anticipate, identify and respond to new and changing needs of our patients
- Travel between patients' homes within a specific geographical region
Qualifications
- Minimum of two-year nursing experience (Community Nursing experience is ideal, but not required)
- Currently registered and in good standing with the College of Nurses of Ontario (CNO)
- Must have completed 'Nurses Specialized in Wound, Ostomy and Continence Canada' (NSWOC) course
- Ability to work independently in managing your own caseload
- Proficiency in English, both verbal and written required (if you can speak additional languages, terrific)
- Valid Ontario driver's license and reliable vehicle with willingness to travel
- Basic proficiency in Word, Excel and ability to learn software
