The Program Manager, Palliative Care will be part of a tight-knit, collaborative team from across the health sector that will deliver the next generation of palliative care in Mississauga, providing holistic proactive, seamless care and support to patients, families and caregivers – enhancing quality of life, comfort, dignity and security in patients' end of life journey. The Program Manager cultivates and manages high-performing teams comprised of Nurses, Personal Support Workers (PSWs), and Allied Health professionals – fostering a collaborative, innovative, and learning-focused environment that aligns with Spectrum's Mission, Vision, and Values.

In this role, you will ensure seamless program operations by leveraging data-driven insights to optimize service delivery, enhance quality, and achieve key outcomes. A critical aspect of your work will involve collaborating with external health system stakeholders to support the design and implementation of innovative programs, ensuring seamless care transitions across the broader health system.

  • Lead program development and design clinical workflows to support innovative models of care while effectively managing key internal and external stakeholder relationships
  • Recruit and onboard team members, ensuring they have the necessary attributes, training and skills – support ongoing development and educational initiatives and development of best-practice champions
  • Lead and mentor the team to consistently meet high care quality standards
  • Collaborate with operational leadership to implement and maintain exemplary practices
  • Address quality, safety, and risk issues, and develop improvement plans as needed
  • Monitor and report key performance indicators, initiating improvements or process changes when required
  • Contribute to improved client outcomes through evidence-informed practices and resource efficiency
  • Integrate client satisfaction plans, addressing concerns in a timely manner.

Qualifications
  • Current CNO certificate as Registered Nurse, degree preferred
  • Minimum of 5 years of recent experience with progressive responsibilities as a Nursing Manager, preferably in a community setting
  • Successful completion of one or a combination of the following required:
    • Canadian Nursing Association (CNA) certification in Palliative Care Nursing or enrollment in CNA certification program in Palliative Care Nursing; or
    • Learning Essentials Approaches to Palliative and End of Life Care (LEAP); or
    • Fundamentals of Palliative Care
    • The Comprehensive Advanced Palliative Care Education (CAPCE)
  • Experience managing in a union environment is an asset
  • Solid knowledge of the principles, practices, and methods related to leading multidisciplinary teams
  • Experience with program development, implementation and evaluation
  • Valid G driver's license and reliable vehicle

  • Demonstrated ability to work collaboratively in a team environment
  • Strong leadership skills, including demonstrated ability and comfort with decision making responsibilities, coaching and teaching, and the ability to inspire and build confidence in others
  • Excellent judgment in setting priorities, identifying issues and determining action required when working under pressure and deadlines
