The Rehabilitative Assistant will provide assistance to our rehab department in providing rehabilitative services to the residents of York Care Centre. They will assist in the development and implementation of treatment plans designed to meet residents' needs all while promoting resident autonomy. They will assist in the modification and adaptation of individual treatment plans in accordance with the residents' status as established by the health care team of professionals. The role of the Rehab Assistant is to improve/maintain the residents' physical wellbeing, and assist in the care planning, establishing goals and benchmarks for the resident.
