We are seeking a permanent full-time Primary Care Nurse Practitioner. This individual must be a forward thinker who can think outside of the box to provide problem solving strategies and contribute to the highest performing Community Health Centre in the Thunder Bay region.
The NP's responsibilities will include:
Working collaboratively with a multi-disciplinary team
Working with the allied health team to develop community-based programs.
Rostering patients and supported by a consulting physician and advanced NPs.
Documenting all encounters on an electronic medical records system ensuring that data quality standards for charting are met.
Experience or willingness to work with patients for addiction medicine
Provide care to a diverse population
Education and knowledge required:
Must have current registration with and be a member in good standing with the College of Nurses of Ontario, with a minimum of 1 years' NP work experience.
Primary health care experience, preferably in a community setting. Program development experience considered an asset.
Advanced proficiency in the use of computers and various software applications including Microsoft Office and electronic medical records systems (Practice Solutions would be considered as asset).
Demonstrated excellent problem-solving, organizational and interpersonal skills with an ability to work effectively and respectfully in a collaborative, interdisciplinary team environment
