CORE RESPONSIBILITIES OF THE POSITION

This position will support the mission, vision and values of the Chatham-Kent Community Health Centres (CKCHC). This position will support safety and the quality of care of clients. The Registered Respiratory Therapist (RRT) will work towards meeting the mandatory performance targets implemented by the Ministry of Health and Long-Term Care. The RRT and Certified Respiratory Educator (CRE) is a member of the primary health care team who provides leadership in the development, administration, implementation and evaluation of respiratory health services, respiratory care & diagnostics and respiratory health education for individuals, families and the community. The RRT provides respiratory care in accordance with the standards of practice as published by the College of Respiratory Therapists of Ontario (CRTO).

Conducts a thorough assessment of individual's health status and knowledge of asthma using the outcomes measurement tools available.

Determines or assists in developing the individual's personal goal(s) regarding asthma and assist the individual attain these.

Informs patients about the goals of therapy and assists in the attainment of these through education and therapeutic intervention.

Provides education on pathophysiology of asthma, medications, proper device technique warning signs, triggers, diaries, and action plans based on education standards.

Demonstrates a thorough knowledge of health and behavior education theories, models and applies these during education sessions.

Assesses and monitors outcomes of education and interventions; revises action plans as appropriate.

Provides smoking cessation counseling in accordance with TEACH principles.

Performs physical assessments, health/education counseling, and client history documenting and other clinical activities according to the CRTO. Performs assessments that are focused and tailored to specific populations.

Participates in community health fairs, provides respiratory health information presentations and represents CKCHC within the community as needed in a variety of venues.

Provides leadership in determining the community needs for respiratory health education programs.

Contributes to building professional and positive working relationships internally and externally.

Collaborate with CKCHC staff and other community agencies to identify, recommend and/or develop resources and materials, and participate in delivery of programs and services.

Accountable for recognizing and obtaining the knowledge, skill and judgement that is needed to meet the needs of the client.

Ensures clinical areas, resources, materials and equipment are maintained.

Collaborate with family physicians to improve respiratory care management for chronic diseases such as COPD and asthma and share results of assessment with family physician.

Facilitate referrals to other services as well as communication and networking between the primary care practice and hospital or home care to ensure smooth transition for medication related care between care sites.

Supervise graduate students in field placement practicum or internships, as requested.

Assists in the achievement of program operational objectives by contributing to strategic planning and review; assisting with the identification and resolution of issues; attending meetings and communicating interdisciplinary clinical concerns; identifying and implementing system improvements.

Demonstrate a commitment to a safe environment for staff, clients and families by working in compliance with the CKCHC related policies, health and safety regulations and completing mandated training and educational sessions as required.

SKILLS, KNOWLEDGE & EXPERIENCE REQUIRED

Minimum 3-5 years of experience in a community setting or combination of community and hospital or public health settings; experience in a primary care setting, an asset.

Demonstrated knowledge and expertise in principles of respiratory conditions; specifically asthma, COPD, smoking cessation, patient counseling and other related respiratory health activities.

Advanced clinical competencies in health assessment, planning and respiratory care delivery.

Uses evidence-based teaching and counseling methods, skills and techniques.

Experience in program development, implantation, monitoring and evaluation.

Proficient in performing and documenting Spirometry as set forth by the Canadian Thoracic Guidelines as is American Thoracic Guidelines.

Expert certified respiratory educator, RRT, case manager and consultant for persons with respiratory conditions.

Possesses the core competencies as identified by the CRTO.

Proficiency in the use of computers and EMR programs is required.

Bilingual (English/French) language skills an asset.

Clean driver's abstract and a current and valid Class "G" Ontario Driver's License with reliable transportation to support travel throughout Chatham-Kent.

Education and certificate requirements

Graduate of an accredited College or University with a Registered Respiratory Therapist (RRT) designation.

Registered and in good standings with the College of Respiratory Therapists of Ontario (CRTO).

Current designation as a Certified Respiratory Educator from the Canadian Network for Respiratory Care (CNRC).

Registered Current BCLS certification.

Knowledge in best care computer program.

Completion of smoking cessation counseling program (TEACH).

Must provide proof of additional insurance coverage as set forth by CRTO.

Completion of a Police Clearance prior to commencing employment.

Full course of COVID-19 Vaccination upon hire (2 doses).
