Detailed Overview:

Provides occupational therapy services and acts as a liaison between the acute care facility and community care service to facilitate discharge planning from the acute care facility to the appropriate service in the community; assesses the patient/client's care needs by completing various assessments including home assessments; advocates for patients/clients with community agencies and funding sources; collaborates with community service providers to provide advice to the patient/client, families and other health care professionals regarding the patient/client's discharge needs.

Care is client-centered, based on goals and outcomes determined in collaboration with the client and caregivers in order to provide safe and quality client care.

Responsibilities:
Provides occupational therapy services to patients/clients through assessment, consultation and interventions that promote functional independence and occupational performance; assesses patient/client's cognitive, perceptual, self care including activities of daily living and/or instrumental activities of daily living, physical and leisure status.
Facilitates timely exchange and provision of information regarding patient/client care between the acute care facility and the community service to allow for individualized discharge planning; works with community service providers to ensure patient/client care needs are met.
Participates in discharge planning rounds in the acute care facility and facilitates timely discharge of patients ensuring appropriate resources are in place; completes pre-discharge home assessments to aid in transition and discharge planning of patients from hospital or prevent admission of clients to hospital. Identifies environmental barriers in the home and makes recommendations for modifications to ensure access for patients/clients with disabilities.
Prescribes and recommends specialized and adaptive aids and equipment to patients/clients for activities of daily living; educates patients/clients/family/caregivers and staff on independent living skills and occupational performance, use of environmental and equipment adaptations, body mechanics, lifts and transfers; develops resource materials as required.
Acts as a patient/client advocate by providing information and liaising with funding agencies and community resources; determines appropriate tasks for delegation; monitors and reviews specific tasks with the patient/client, caregiver and/or home support worker to ensure a high standard of care and patient/client safety.
Completes and maintains complete documentation of patient/client information such as forms, charts, records, statistical information and other related information, as required.
Participates in quality improvement activities of the program by providing input into the development of indicators, standards, practices and procedures; provides recommendations to the Manager.
Maintains knowledge in current developments in evidence-based occupational therapy and practice by reviewing relevant literature, consulting with other health care professionals and attending relevant educational programs.
Performs other related duties as assigned.
Qualifications: Education and Experience

Current full registration with the College of Occupational Therapists of British Columbia (COTBC). Two (2) years' recent, related clinical experience in a community or home health setting, or an equivalent combination of education, training and experience.

Valid BC Driver's License and access to personal vehicle for business-related purposes.
