THE ROLE

As a member of our healthcare field team, the position will involve providing care to residents of our partnered homes, and providing support to the permanent staff of the homes. We have homes all across Stratford and KW, and so opportunities may involve driving up to a maximum of 40 minutes. There are additional opportunities for further travel, that may involve additional compensation.

All staff will receive MULTIPLE training opportunities prior to working independent in homes, and will participate in our Policies and Procedures session to ensure that they are aware of all the ways that Plan A offer support and guidance prior to being placed.

Reports to: The Charge Nurse of the homes, as well as Plan A Scheduler and Director of Operations

RESPONSIBILITIES

Ensure Resident care is provide with established nursing standards and principles, physicians' orders and administrative policies, as well as, observe, report and record vital signs, symptoms and conditions of Residents, and initiate remedial health measures where appropriate. Assure observance of required techniques in nursing care.
Works in areas which may involve Residents with social behavioural changes.
Maintaining positive relationship with all Residents, family members, physicians, nursing and dietary staff.
Plan, direct, supervise and evaluate the work of nursing and personal care staff assigned to the unit. Orient and train new personnel and may participate in in-service education.

Dispenses medications using electronic documentation system.
Assess Residents and develop, implement, review and evaluate Resident care plans. Monitor Residents' health, notes changes in condition and assess need for referral to doctors; processes and implement physician orders.
Any other duties as assigned.

EDUCATION AND EXPERIENCE

At least 2-3 years of experience working in a healthcare setting
Must have successfully completed a Nursing program and hold a valid CNO license that is in good standing.

Experience in Long-Term Care considered an asset

Clear Criminal Record with Vulnerable Sector Check, dated from within the past 6 months.

Strong interpersonal and customer focus skills
Must have demonstrated sound skills and knowledge of providing personal care duties
Ability to perform tasks that are necessary for the position, e.g. lifting/transferring clients
