Overview

We are currently looking for someone to join our team in the role of Program Manager, Provincial Integrated Care. This position is based out of our Waterloo, ON corporate office, with hybrid working options available.

The Program Manager, Provincial Integrated Care is responsible on a provincial level for coordinating all the integrated care programs, projects, and activities for our organization. This role will be tasked with identifying opportunities, developing programs and models to support the needs of the sector and organization's strategic direction, as well as creating and managing long term goals of the integrated programs. The Program Manager will also be accountable for developing budgets and operating plans for programs and writing program funding proposals. They will collaborate closely with the operational leaders and team members to ensure alignment and overall management of the programs are running smoothly.

What The Role Involves
  • Organizing and coordinating all integrated care programs and activities in accordance with the organization's mission and goals.
  • Developing new integrated care programs and models to support the strategic direction of the organization.
  • Creating and managing long-term organization and program goals.
  • Developing and managing the budget and operating plan for the integrated programs.
  • Developing an evaluation method to assess program strengths and identify areas for improvement.
  • Writing effective program funding proposals to secure and/or expand business opportunities and programs.
  • Managing a team with a diverse array of talents and responsibilities.
  • Ensuring goals are met in areas including client, funder and staff experience, safety, quality, and team member performance.
  • Implementing and managing changes and interventions to ensure project goals are achieved.
  • Meeting with stakeholders to facilitate effective and transparent communications regarding project issues and decisions on services.
  • Producing accurate and timely reporting of program status throughout its life cycle.
  • Working with operational leaders to identify, secure, and onboard resources for programs.
  • Tracking progress against project and program milestones.
  • Scheduling internal and external program meetings while also tracking and following-up with action items.
  • Identifying barriers or risks to successful program execution and escalating as appropriate.
  • Building partnerships with various industry stakeholders and leaders.
  • Understanding contracts and agreements with funders to ensure all requirements are fulfilled.
  • Managing subcontractor relationships for integrated care programs.
What You Bring
  • Bachelor's degree or master's degree in business, health, or related field.
  • Minimum 5 years of health care related experience.
  • PMP certification or experience in project management.
  • Demonstrated knowledge of change management principles and performance evaluation.
  • Demonstrated knowledge of project management tools, techniques and methodologies.
  • Proven experience in program management.
  • Proven stakeholder management skills.
  • Proven experience managing a team.
  • Strong organization, communication and problem-solving skills.
  • Strong influencing and negotiation skills.
  • Strong presentation skills.
  • Excellent knowledge of MS Office; working knowledge of program/project management software (MS Project etc.) is a strong advantage.
  • Valid driver's license, own vehicle and appropriate insurance.
