The Care Navigator / Neighborhood Manager plays a vital leadership role within our neighborhood-based care model. This position ensures high-quality, resident-centered care while supporting staff development, interdisciplinary collaboration, and overall neighborhood performance.
WHAT YOU'LL DO
Resident-Focused Care
· Ensure residents' physical, emotional, social, and spiritual needs are consistently met.
· Monitor resident care and escalate clinical concerns to the ERCNM as needed.
· Set up internal transfers and ensure smooth transitions.
· Coordinate and follow through on all admissions and respite placements.
· Lead and follow up on Advance Care Directives (ACDs), collaborating with SDMs and POAs.
· Initiate and facilitate care conferences.
· WanderGuard alerts and systems updates.
· Support residents with dementia through P.I.E.C.E.S and/or referrals to behavioral specialists.
· Serve as a primary contact for families, ensuring concerns are addressed from admission through the continuum of care.
· Maintain strong communication and relationships with Yarmouth Regional Hospital, EHS, and other healthcare partners.
· Participate in and support the Resident Family Council.
· Provide updates to families as needed and ensure ongoing engagement.
Leadership & Staff Management:
· Provide mentorship, training, and support to nursing and neighborhood team.
· Assist with recruitment and performance and neighborhood goal setting
· Train and develop new staff, referring concerns to the DOC/ADOC/Department Managers.
· Provide support and needs for staff on best practices in resident care and resident-centered care.
· Promote continuous learning and professional growth among the care team
· Lead huddles, problem-solving and team meetings
· Cover for other roles during absences (i.e. ADOC, Care Navigator, Infection Control)
Compliance & Quality Assurance:
· Ensure adherence to licensing requirements
· Conduct audits and quality improvement reviews.
· Support leadership in policy implementation and workforce planning.
· Works in accordance with the organization's Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act/Regulations.
WHAT YOU NEED TO HAVE
· Regulated Health Professional of Nova Scotia (for e.g. Registered Nurse, Social Worker, Occupational Therapist, Physiotherapist, Dietitian, or similar designation in good standing.)
· 3 years leadership/management experience
· Proven leadership experience and a commitment to continuous learning.
· Willingness to upgrade and maintain qualifications as required by evolving job demands.
· Ability to work harmoniously within a multidisciplinary team, lead and support the Neighborhood Team, and uphold the organization's values and philosophy.
· Adherence to professional competencies, including caring and respectful behavior, therapeutic relationships, strong clinical and assessment skills, and communication skills
· Self-directed, resilient to change, and capable of facilitating, resolving conflicts, and leading projects and programs. While maintaining excellent organizational, analytical, and time management skills.
· Reliability and availability to attend work.
· Well-organized, able to initiate workload independently, while collaborating closely with the Neighborhood Team, Director of Care, managers, and other departments.
· Commitment to promoting and supporting a positive work environment while maintaining strict confidentiality.
· Excellence in computer skills, including experience with Inter-Rai /Civica/Momentum
· Agreement to have the role revised and restructured based on operational requirements.
· Ability to manage conflicts effectively and build teams
· Commitment to employment equity, diversity, and inclusion in the workplace.
WHAT WE'D LOVE YOU TO HAVE
· Long Term Care experience
· Possession of valid CPR, and First Aid Certificates.
· Leadership courses (Team building Training / People Centered Training)
· Quality Systems methodology training and experience (i.e lean)
· Familiar working in a unionized environment
