Status: Full-time Term
Term: One year
Hours of work: 75 hours bi-weekly, Monday to Friday
Salary: Dependent on education and experience

The mandate of the Eel Ground Health and Wellness Center is to ensure the health needs of the community are met through the design, implementation and delivery of proactive health care services, initiatives and programs.

The Outreach Support Worker will work under supervision of the Health Director. The role of the Outreach Support worker is to provide assistance, guidance and support to individuals facing various challenges. The position will work directly with the health centre's harm reduction team.

Primary Areas of Responsibility

  • Build and maintain strong relationships with clients, providing empathetic support and advocacy.
  • Conduct outreach activities to connect with individuals in need within the community.
  • Assess clients' needs and develop personalized action plans.
  • Provide referrals to appropriate services, including housing, healthcare, education, and employment support.
  • Collaborate with community organizations, agencies, and stakeholders to coordinate resources and programs.
  • Distribution of harm reduction supplies.
  • Provide education to the public through community engagement sessions.

Qualifications:

  • Training and/or education in a related human/social service field.
  • Previous experience in outreach (preferred but not required).
  • Strong interpersonal and communication skills.
  • Ability to work independently and within a team setting.
  • Working knowledge of relevant community resources in the local area.
  • Current First Aid Certificate required for employment

Preferred Experience:

  • Knowledge of Harm Reduction
  • Knowledge of HIV/AIDS, Hepatitis C, and other STBBIs
  • Familiarity with issues around addictions and/or people who use drugs
  • Familiarity with community resources; particularly harm reduction, addiction, mental health, and others
  • Naloxone Training, Crisis Intervention (CPI), and Trauma Informed Practice Training is an asset
  • Ability to work with disadvantaged and challenging adults in a diverse environment
  • Strong ability to manage stress and organize workload
  • Understands and maintains clientele/worker boundaries
  • Willing to work after hours
