Job Summary: The Program Coordinator is responsible for the development, implementation, and supervision of the Life Skills Program at HIL and the affiliated Outreach Program. The Program Coordinator works with each participant to develop individual program plans and provides direction, guidance, and support to the Front-Line Staff team to implement such programs.

Major Responsibilities:

Program Development/Implementation/Evaluation and Client Services (50%)

Ensure proper placement of individuals in the Program through running assessments and making recommendations.

Develop Individual Program Plans for Participants and Outreach Clients.

Support and direct participants to ensure they are gaining necessary skills to increase their independence.

Lead programming meetings and participant meetings to evaluate/set goals, address areas of concern, and make changes where necessary.

Documentation and report writing for Participants and Outreach Clients.

Maintain communication with the families, support networks, and professionals involved with the Participants and Outreach Clients.

Organizational Operation/Office Administration (30%)

Be part of the on-call rotation.

Cover responsibilities of the Executive Director when not present.

Complete staff scheduling and training.

Assist with hiring and retention of competent and qualified staff.

Maintain office supplies and household groceries as needed.

Complete all courses to perform safety inspections monthly.

Develop and review policies required for participants and outreach clients.

Complete biweekly payroll and monthly reconciliation.

Government/Community Liaison and Advocate (20%)

Maintain positive and effective relationships with Care Coordinators, Medical Professionals, and Service Providers.

Attend professional appointments/meetings with Participants and Outreach Clients when needed.

Advocate in various settings on behalf of participants and teach participants to become their own advocates.

Qualifications:

Education/Certifications:

Undergraduate Degree in Social Sciences/Human Services related field.

Standard First Aid/CPR, Non-Violent Crisis Intervention, Food Handlers, WHMIS.

Seven Core Competencies as set by the Department of Community Services/Services for Persons with Disabilities (must be completed within the first six months of hire).

Experience:

Minimum of 5 years of experience supporting persons with disabilities.

Minimum of 3 years of experience in a leadership role.

Strong knowledge of program development and implementation.

Previous experience in a supervisory position.

Strong interpersonal and communication skills.

Other Requirements:

Ability to work flexible hours, including one evening shift a week and one weekend shift a month.

Must be in the city every three years for Christmas coverage, as part of a rotation with management personnel at HIL.

Reporting Relationship: This position reports to the Executive Director.

Direct Reports: This position is not responsible for human resources but has 11 to 14 staff reporting to them about programming.
